目的:观察芪参益气滴丸对稳定型心绞痛患者经皮冠状动脉介入治疗(PCI)围手术期心肌损伤的影响。方法选择2012年1月至2013年6月稳定型心绞痛行择期PCI治疗患者101例,入院后随机分为芪参益气滴丸组(芪参组,n=51)和常规治疗组(对照组,n=50)。对照组予以冠心病常规治疗,芪参组在对照组的基础上加用芪参益气滴丸0.5g,每日3次,入院后即开始服用。3~7(5.5±1.5)天后行PCI治疗。入院后次日晨及PCI术后次日晨测定血浆肌钙蛋白I(TnI)和肌酸激酶同工酶(CK-MB)浓度,观察PCI前后血浆TnI和CK-MB水平的变化。结果PCI术前芪参组和对照组TnI和CK-MB浓度无显著差异(P>0.05),术后对照组TnI和CK-MB浓度较术前明显升高(P<0.01),芪参组TnI和CK-MB浓度虽有升高趋势,但与术前相比无明显差异(P>0.05),且芪参组术后TnI和CK-MB浓度均明显低于对照组(P<0.05)。结论芪参益气滴丸能减轻稳定型心绞痛患者PCI围手术期心肌损伤。

목적:관찰기삼익기적환대은정형심교통환자경피관상동맥개입치료(PCI)위수술기심기손상적영향。방법선택2012년1월지2013년6월은정형심교통행택기PCI치료환자101례,입원후수궤분위기삼익기적환조(기삼조,n=51)화상규치료조(대조조,n=50)。대조조여이관심병상규치료,기삼조재대조조적기출상가용기삼익기적환0.5g,매일3차,입원후즉개시복용。3~7(5.5±1.5)천후행PCI치료。입원후차일신급PCI술후차일신측정혈장기개단백I(TnI)화기산격매동공매(CK-MB)농도,관찰PCI전후혈장TnI화CK-MB수평적변화。결과PCI술전기삼조화대조조TnI화CK-MB농도무현저차이(P>0.05),술후대조조TnI화CK-MB농도교술전명현승고(P<0.01),기삼조TnI화CK-MB농도수유승고추세,단여술전상비무명현차이(P>0.05),차기삼조술후TnI화CK-MB농도균명현저우대조조(P<0.05)。결론기삼익기적환능감경은정형심교통환자PCI위수술기심기손상。
Objective To observe the influence of Qishen Yiqi Gutta Pills on perioperation myocardial damage in the patients with stable coronary heart disease (CHD) after percutaneous coronary intervention (PCI). Methods The patients (n=101) with selected PCI were chosen from Jan. 2012 to Jun. 2013 and randomly divided into Qishen Yiqi Gutta Pills group (Qishen group, n=51) and routine treatment group (control group, n=50). Control group was treated with routine theray of CHD and Qishen group, with Qishen Yiqi Gutta Pills (0.5 g 3 times a day) besides of routine treatment immediately after hospitalizaton. After 3 d to 7 d (5.5±1.5) the patients were given PCI. The concentration of plasma TnI and CK-MB were detected respectively the next morning after hospitalization and the next morning after PCI. The changes of plasma TnI and CK-MB were observed before and after PCI. Results There was no significant difference in the concentration of plasma TnI and CK-MB in two groups before and after PCI (P>0.05). After PCI, the concentration of plasma TnI and CK-MB increased significantly in control group (P<0.01), and had increasing trend in Qishen group but was not significantly different (P>0.05). The concentration of TnI and CK-MS was significantly lower in Qishen group than those in control group after PCI (P<0.05). Conclusion Qishen Yiqi Gutta Pills can alleviate the myocardial damage in the patients with stable CHD during perioperation of PCI.
