目的：探讨产科应用舒芬太尼复合左旋布比卡因进行麻醉的临床效果。方法收集2011年6月~2013年6月于瑞安市人民医院收治的行剖宫产孕妇118例，随机分为研究组和对照组，每组各59例。对照组给予生理盐水+0.5%左旋布比卡因麻醉，研究组在对照组基础上予以舒芬太尼，比较两组的麻醉起效时间、镇痛效果、不良反应等项目。结果①两组最高感觉阻滞平面、运动阻滞起效时间、Bromage评分差异无统计学意义(P>0.05)；研究组达到最高阻滞平面时间[(8.5±1.9)min]、达最大运动阻滞时间[(3.7±1.3)min]短于对照组[(9.8±1.8)、(5.0±1.1)min]，感觉阻滞恢复时间[(273.8±28.0)min]、运动阻滞完全恢复时间[(237.6±26.9)min]长于对照组[(247.1±27.6)、(218.3±27.5)min]，差异均有高度统计学意义(P<0.01)。②研究组镇痛分级比例优于对照组，差异有统计学意义(P<0.05)。③研究组低血压发生率[3.4豫(2/59)]低于对照组[16.9豫(10/59)]，差异有统计学意义(P<0.05)；皮肤瘙痒、心动过缓、恶心呕吐等并发症发生率两组差异无统计学意义(P>0.05)。两组新生儿出生后1、5 min的Apgar评分差异无统计学意义(P>0.05)。结论产科应用舒芬太尼复合左旋布比卡因具有起效快、镇痛效果明显的特点，值得临床推广。

목적：탐토산과응용서분태니복합좌선포비잡인진행마취적림상효과。방법수집2011년6월~2013년6월우서안시인민의원수치적행부궁산잉부118례，수궤분위연구조화대조조，매조각59례。대조조급여생리염수+0.5%좌선포비잡인마취，연구조재대조조기출상여이서분태니，비교량조적마취기효시간、진통효과、불량반응등항목。결과①량조최고감각조체평면、운동조체기효시간、Bromage평분차이무통계학의의(P>0.05)；연구조체도최고조체평면시간[(8.5±1.9)min]、체최대운동조체시간[(3.7±1.3)min]단우대조조[(9.8±1.8)、(5.0±1.1)min]，감각조체회복시간[(273.8±28.0)min]、운동조체완전회복시간[(237.6±26.9)min]장우대조조[(247.1±27.6)、(218.3±27.5)min]，차이균유고도통계학의의(P<0.01)。②연구조진통분급비례우우대조조，차이유통계학의의(P<0.05)。③연구조저혈압발생솔[3.4예(2/59)]저우대조조[16.9예(10/59)]，차이유통계학의의(P<0.05)；피부소양、심동과완、악심구토등병발증발생솔량조차이무통계학의의(P>0.05)。량조신생인출생후1、5 min적Apgar평분차이무통계학의의(P>0.05)。결론산과응용서분태니복합좌선포비잡인구유기효쾌、진통효과명현적특점，치득림상추엄。
Objective To analyze the analgesic effect of Sufentanil combined with Levobupivacaine in obstetrical de-partment. Methods 118 pregnant woman with cesarean delivery, from June 2011 to June 2013 in the People's Hospital of Rui'an City, were selected and divided into research group and control group randomly, with 59 cases in each group. The control group were given with normal saline+0.5% Levobupivacaine, the research group were given with Sufentanil on the basses of research group. The anesthesia work time, analgesic effect, untoward effect were compared between the two groups. Results ①The differences of the highest sensory blockade plane, motion block work time, Bromage scores between the two groups was not statistically significant (P>0.05). The achievement time of the highest block plane, the achievement time of biggest sports block in research group [(8.5±1.9), (3.7±1.3) min] were all shorter than those in con-trol group [(9.8±1.8), (5.0±1.1) min], the sensory blockade recovery time, motion block completely recovery time in re-search group [(273.8±28.0), (237.6±26.9) min] were all longer than those in control group [(247.1±27.6), (218.3±27.5) min], the differences were statistically significant (P< 0.01). ②The analgesia grade in research group was better than those in control group, the differences were statistically significant (P< 0.05). ③The incidence of hypotension in re-search group [3.4% (2/59)] was lower than that in control group [16.9% (10/59)], the difference was statistically signifi-cant (P< 0.05); the difference of incidence of cutaneous pruritus, bradycardia, nausea and vomiting between the two groups was not statistically significant (P>0.05). The differences of the Apgar scores of the newborn 1, 5 min after the birth between the two groups was not statistically significant (P>0.05). Conclusion The application of Sufentanil com-bined with Levobupivacaine in obstetrical department has rapid onset, obvious analgesic effect, it is worthy of clinical popularization.
