目的:观察健脾活血补肾汤体外对强直性脊柱炎(AS)患者外周血单个核细胞IL-23与IL-17表达的影响,从而探讨健脾活血补肾汤治疗AS的可能机制。方法:制备不同浓度健脾活血补肾汤大鼠含药血清,用ELISA方法检测经不同浓度大鼠含药血清培养的AS患者和健康志愿者PBMCs上清液中IL-23与IL-17水平。结果:AS患者PBMCs经72、36 g·kg-1·d-1两组浓度大鼠含药血清培养后,与生理盐水组及18 g·kg-1·d-1组相比,上清液中IL-23与IL-17水平显著下降(P<0.01),但健脾活血补肾汤组含药大鼠血清对健康自愿者PBMCs上清液中IL-23与IL-17表达水平影响均没有统计学意义。结论:健脾活血补肾汤可能通过下调炎症因子IL-23和IL-17的表达而起到延缓关节炎性骨质破坏的作用。

This study was aimed to observe in v itro IL-23 and IL-17 expression in peripheral blood mononuclear cell (PBMC) among ankylosing spondylitis (AS) patients treated with Jian-pi Huo-xue Bu-shen (JPHXBS) decoc-tion, in order to explore the possible treatment mechanism of AS by JPHXBS decoction. Different concentrations of rat serum containing JPHXBS decoction were prepared. ELISA was used in the detection of PBMCs supernatant IL-23 and IL-17 levels from AS patients and healthy volunteers cultivated rat serum with different medication concen-trations. The results showed that PBMCs of AS patients treated with different concentrations (72 and 36 g·kg-1·d-1) of rat serum containing incubation, the supernatant IL-23 and IL-17 levels were significantly decreased compared with the normal saline group and the group of 18 g·kg -1·d -1 (P < 0.01). However, there was no statistical significance on supernatant IL-23 and IL-17 expression in PBMCS between JPHXBS decoction group and healthy volunteers. It was concluded that JPHXBS decoction may downregulate the inflammatory factors of IL-23 and IL-17 expression in order to delay articular inflammatory bone destruction.
