背景:自体血回输可有效提供相容性血细胞,及时补充血容量,减少异体血输入量,降低异体输血的并发症,对机体的凝血功能无不良影响,因而越来越受到人们的重视。目前,有关髋关节手术应用该技术的临床研究甚少。<br> 目的:观察髋关节手术中自体血回输技术(IOCS)的临床应用效果。<br> 方法:选择40例髋关节手术患者,随机分为自体血回输组(实验组)20例和输注异体血组(对照组)20例。观察不同时段血常规、凝血时间的变化以及并发症发生情况。<br> 结果:实验组比对照组共节约库存用血约8000 ml。A组患者自体血回输即刻与术前相比血红蛋白(Hb)降低,但很快恢复正常,4 d后红细胞计数(RBC)、Hb、红细胞压积(Hct)与术前相比无统计学差异。血小板计数(PLT)及凝血酶原时间(PT)与术前相比有统计学差异(P<0.05),与B组相比无统计学差异(P>0.05)。<br> 结论:IOCS是一种安全有效的节约用血方法,适合在骨科髋关节手术中推广。

Background:It has been approved that intraoperative blood autologous transfusion can provide efficient hemocytes, supply blood volume,and decrease bank blood transfusion and complications. <br> Objective:To evaluate the clinical outcome of intraoperative cell salvage during hip joint surgery. <br> Methods:Forty patients undergoing hip joint surgery were randomly divided into experiment group and control group (n=20). Patients received intraoperative blood salvage and bank-blood, respectively, in the experiment group and control group. Blood routine examination, coagulation time and complications were recorded. <br> Results:The blood volume consumpted during surgery in experiment group was less than 8000 ml than control group. The hemoglobin decreased when autologous transfusion started,while it took back normal level quickly. Red blood count, hemo-globin and hematocrit at 4 days after surgery were not significantly different from preoperative ones, while blood platelets count and prothrombin time at 4 days after surgery were significantly different from preoperative ones (P<0.05). <br> Conclusions:Intraoperative cell salvage is safe and effective for decrease of blood transfusion and can be widely used dur-ing hip joint surgery.
