[目的]探讨浓缩自体骨髓移植结合金葡液注射治疗骨质疏松性骨折的疗效及其可能的分子机理。[方法]5月龄雌性家兔80只,体重2.5~3.0kg,采用去势法建造OPF动物模型,随机分成4组。分别为浓缩自体骨髓移植结合金葡液局部注射治疗组(A组)、浓缩自体骨髓移植治疗组(B组)、金葡液局部注射治疗组(C组)、未治疗组(D组),每组20只。分四个时间点(2W、4W、6W、8W)处死动物取材,进行X线摄片、测定骨痂CT值、BMP-2及VEGF染色阳性细胞计数观察。[结果]各取材时间A组X线表现明显优于其它三组,A组第8W时骨折线基本消失。CT值对比显示各组CT值呈递增趋势,A组递增幅度最大,D组递增幅度最小,同一时间点取材各组骨痂CT值比较发现A组骨痂CT值均高于其它组(P<0.01)。HE染色显示A组在2、4W时软骨细胞、成骨细胞较多,6、8W取材时可见细胞都已经成熟;D组第8W时仍较多软骨细胞存在。免疫组化结果显示2W时A组骨痂细胞中BMP-2、VEGF的表达量均高于其它组;第8W时A、B、C组表达量均极少,D组表达量高于前3组。[结论]浓缩自体骨髓移植结合金葡液局部注射能促进骨质疏松性骨折愈合,其作用明显优于单纯应用骨髓移植治疗或单纯金葡液局部注射治疗,其机理可能是提高了骨折早中期BMP-2和VEGF的表达。
[目的]探討濃縮自體骨髓移植結閤金葡液註射治療骨質疏鬆性骨摺的療效及其可能的分子機理。[方法]5月齡雌性傢兔80隻,體重2.5~3.0kg,採用去勢法建造OPF動物模型,隨機分成4組。分彆為濃縮自體骨髓移植結閤金葡液跼部註射治療組(A組)、濃縮自體骨髓移植治療組(B組)、金葡液跼部註射治療組(C組)、未治療組(D組),每組20隻。分四箇時間點(2W、4W、6W、8W)處死動物取材,進行X線攝片、測定骨痂CT值、BMP-2及VEGF染色暘性細胞計數觀察。[結果]各取材時間A組X線錶現明顯優于其它三組,A組第8W時骨摺線基本消失。CT值對比顯示各組CT值呈遞增趨勢,A組遞增幅度最大,D組遞增幅度最小,同一時間點取材各組骨痂CT值比較髮現A組骨痂CT值均高于其它組(P<0.01)。HE染色顯示A組在2、4W時軟骨細胞、成骨細胞較多,6、8W取材時可見細胞都已經成熟;D組第8W時仍較多軟骨細胞存在。免疫組化結果顯示2W時A組骨痂細胞中BMP-2、VEGF的錶達量均高于其它組;第8W時A、B、C組錶達量均極少,D組錶達量高于前3組。[結論]濃縮自體骨髓移植結閤金葡液跼部註射能促進骨質疏鬆性骨摺愈閤,其作用明顯優于單純應用骨髓移植治療或單純金葡液跼部註射治療,其機理可能是提高瞭骨摺早中期BMP-2和VEGF的錶達。
[목적]탐토농축자체골수이식결합금포액주사치료골질소송성골절적료효급기가능적분자궤리。[방법]5월령자성가토80지,체중2.5~3.0kg,채용거세법건조OPF동물모형,수궤분성4조。분별위농축자체골수이식결합금포액국부주사치료조(A조)、농축자체골수이식치료조(B조)、금포액국부주사치료조(C조)、미치료조(D조),매조20지。분사개시간점(2W、4W、6W、8W)처사동물취재,진행X선섭편、측정골가CT치、BMP-2급VEGF염색양성세포계수관찰。[결과]각취재시간A조X선표현명현우우기타삼조,A조제8W시골절선기본소실。CT치대비현시각조CT치정체증추세,A조체증폭도최대,D조체증폭도최소,동일시간점취재각조골가CT치비교발현A조골가CT치균고우기타조(P<0.01)。HE염색현시A조재2、4W시연골세포、성골세포교다,6、8W취재시가견세포도이경성숙;D조제8W시잉교다연골세포존재。면역조화결과현시2W시A조골가세포중BMP-2、VEGF적표체량균고우기타조;제8W시A、B、C조표체량균겁소,D조표체량고우전3조。[결론]농축자체골수이식결합금포액국부주사능촉진골질소송성골절유합,기작용명현우우단순응용골수이식치료혹단순금포액국부주사치료,기궤리가능시제고료골절조중기BMP-2화VEGF적표체。
[Objective]Explore the effect of concentrated autologous bone marrow implanted and local injection with Staphylococcal Enterotoxin C Injec-tion and its possible biological mechanism in treating osteoporotic fracture(OPF).[Methods] A total of 80 female rabbits,5-month-old,weighing 2.5~3.0 kg, were subjective to ovary resection to establish osteoporotic fracture(OPF) model, and were randomly divided into 4 groups, concentrated autologous bone marrow implanted and local injection with Staphylococcal Enterotoxin C Injection group(group A), concentrated autologous bone marrow implanted group(group B), local injection with Staphylococcal Enterotoxin C Injection group(group C), no treatment group(group D),with 20 rabbits in each group. Observe bones through X-ray, CT value and calculate BMP-2 and VEGF positive cells after the rabbits were sacrificed in 2, 4 , 6 and 8 weeks. [Results] X-ray indicated fracture line of group A faded more quickly than other 3 groups and disappeared in 8 weeks. CT value of cal us in each group gradual y increased in different time,especial y in group A and the smal est account in group D.But in the same time, CT value of cal us of group A was obviously higher than that of other groups(P<0.01). HE demonstrated that there were many chondrocytes and osteoblasts in 2 and 4 weeks,which grew mature in 6 and 8 weeks, however group D stil had more chondrocyte in 8 weeks.SABC displayed the expression of BMP-2 and VEGF of cal us cells in group A was higher than other three groups in 2 weeks; but that of group D was the highest in 8 weeks,which had fewer expression in groups A,B and C. [Con-clusion] Concentrated autologous bone marrow implanted combining with local injection of Staphylococcal Enterotoxin C can speed up the healing of OPF,its effect is obviously better than that of either method. Its possible biological mechanism is increasing the expression of BMP-2 and VEGF of cal us cells in early and middle periods.
