[目的]探讨2型糖尿病（type 2 diabetes mel itus, T2DM）中医证型与自身免疫性甲状腺疾病的关系，指导T2DM患者筛查甲状腺自身抗体。[方法]选取T2DM阴虚热盛型、湿热困脾型、气阴两虚型、血瘀脉络型及阴阳两虚型患者各30例，对照组健康人30例，测定甲状腺功能及抗体，统计分析中医各证型与甲状腺功能及抗体间的关系。[结果]T2DM甲状腺功能减退患病率高于对照组，阴阳两虚型患者甲状腺功能减退患病率高于其余4型，阴虚热盛型患者TPOAb阳性率高于其余4型，差别均有统计学意义(P<0.05)。[结论]T2DM尤其是阴虚热盛型患者筛查甲状腺自身抗体对T2DM合并自身免疫性甲状腺疾病的早期诊断具有重要意义，且TPOAb检查意义大于TGAb。
[목적]탐토2형당뇨병（type 2 diabetes mel itus, T2DM）중의증형여자신면역성갑상선질병적관계，지도T2DM환자사사갑상선자신항체。[방법]선취T2DM음허열성형、습열곤비형、기음량허형、혈어맥락형급음양량허형환자각30례，대조조건강인30례，측정갑상선공능급항체，통계분석중의각증형여갑상선공능급항체간적관계。[결과]T2DM갑상선공능감퇴환병솔고우대조조，음양량허형환자갑상선공능감퇴환병솔고우기여4형，음허열성형환자TPOAb양성솔고우기여4형，차별균유통계학의의(P<0.05)。[결론]T2DM우기시음허열성형환자사사갑상선자신항체대T2DM합병자신면역성갑상선질병적조기진단구유중요의의，차TPOAb검사의의대우TGAb。
[Objective] To study the relationship between type 2 Diabetes(T2DM) in different TCM syndromes and autoimmune thyroid disease(AITD), guide autoimmune thyroid antibodies screening. [Methods] Select 30 cases for every syndrome including yin deficiency and excess heat syndrome ,damp heat syndrome, qi and yin deficiency syndrome,deficiency of yin and yang syndrome and stagnated blood syndrome. 30 cases of healthy people were selected as the normal control group(NCG). Thyroid function and autoimmune thyroid antibodies were measured and the relationship between TCM syndrome and thyroid function and autoimmune thyroid antibodies were analysed. [Results] The prevalence of hypothyroidism of T2DM group was higher than that of NCG, the prevalence of hypothyroidism of deficiency of yin and yang syndrome was higher than other four syndromes, the positive rates of TPOAb of yin deficiency and excess heat syndrome were higher than other syndromes ,with a significant difference. [Conclusion] Examining autoimmune thyroid antibodies of T2DM especial y of yin deficiency and excessive heat syndrome has great importance to early diagnosis and treatment of T2DM with AITD.
