目的:探讨高频超声在早期妊娠中的诊断价值。方法对不同孕周的1800名早孕妇女分三组给予经腹常规超声及高频超声扫查。比较高频超声与常规超声对各组妊娠囊(GS)、卵黄囊(YS)及胚胎组织的显示能力。对另300名早孕孕妇同时行经阴道及经腹部高频超声检查,比较经腹高频超声与经阴道超声(TVS)显示GS 、YS及胚胎组织的情况。结果高频超声与常规超声对GS、YS及胚胎显示率的比较,在孕龄小于4周及4~7周时,差异有统计学意义(P<0.001);在孕周大于等于7周时,差异无统计学意义(P>0.1)。高频超声和TVS对GS、YS及胚胎组织三项指标的显示率比较,差异无统计学意义(P>0.1)。结论高频超声可以明显提高小于7周的早孕GS、YS及胚胎组织的显示率,与TVS类似,且图像清晰,操作简便,值得临床应用。

목적:탐토고빈초성재조기임신중적진단개치。방법대불동잉주적1800명조잉부녀분삼조급여경복상규초성급고빈초성소사。비교고빈초성여상규초성대각조임신낭(GS)、란황낭(YS)급배태조직적현시능력。대령300명조잉잉부동시행경음도급경복부고빈초성검사,비교경복고빈초성여경음도초성(TVS)현시GS 、YS급배태조직적정황。결과고빈초성여상규초성대GS、YS급배태현시솔적비교,재잉령소우4주급4~7주시,차이유통계학의의(P<0.001);재잉주대우등우7주시,차이무통계학의의(P>0.1)。고빈초성화TVS대GS、YS급배태조직삼항지표적현시솔비교,차이무통계학의의(P>0.1)。결론고빈초성가이명현제고소우7주적조잉GS、YS급배태조직적현시솔,여TVS유사,차도상청석,조작간편,치득림상응용。
Objective To evaluate the diagnostic efficacy of high-frequency ultrasound(US)in early pregnancy.Methods 1800 pregnant women of different gestational age underwent routine and high-frequency trans-abdominal US.The image quality of gestational sac(GS),yolk sac(YS)and embryo was compared.300 pregnant women also underwent trans-vaginal US examination and the image quality of GS,YS and embryo was compared with trans-abdominal high-frequency US.Results The image quality of GS, YS and embryo was significantly better on high-frequency than conventional trans-abdominal US at gestational age of less than 4 weeks and at 4-7 weeks(P<0.001).At gestational age≥7 weeks,there was no significant difference(P>0.100).There was no significant difference between tran-vaginal US and high-frequency US(P>0.100).Conclusion High-frequency US can improve visibility of GS,YS and embryo at less than 7 weeks of gestation similar to trans-vaginal US with good image quality and easy operability.
