目的:探讨局部点压技术在乳腺钼靶摄影中的应用价值及其使用原则。方法对156例经常规乳腺钼靶摄影判断存在可疑病变患者,辅以局部点压摄片,将获得的影像信息与病理结果进行比较分析,并统计常规乳腺钼靶摄影及其联合局部点压摄片的辐射剂量。结果常规钼靶摄影时,病变的检出率为85.3%,诊断特异性为79.5%;加摄局部点压片后病变检出率为94.2%,诊断特异性为86.5%。两种摄片方法在辐射剂量方面无统计学差异。结论使用局部点压技术能提高钼靶摄影对乳腺疾病诊断的检出率与特异性,尤其适用于隐匿部位的小病灶和致密型乳腺的X线检查,但应用过程中要严格掌握其适应证。

Objective To explore the application value and applying rules of local spot compression technique in mammography. Methods 156 patients with suspicious lesions examined through mammography were tested by using local spot compression photography. The diagnosis results of mammography combined with local spot compression photography were analyzed and compared with the pathological results. The radiation dose of mammography combined with local spot compression photography was also compared with that of mammography. Results The sensitivity and specificity of mammography were 85.3% and 79.5% while those of mammography combined with spot local compression photography were 94.2% and 86.5%. There was no significant difference between the radiation dose of mammography and that of mammography combined with local spot compression photography. Conclusion The detection rate and diagnosis accuracy of mammography for breast diseases can be improved with the local spot compression technique, which is especially suitable for the X-ray examination of small lesions exsit in hidden parts and compact breasts. But we should pay attention to the indications during the using process of the local spot compression technique.
