目的:探讨T1ρ对膝关节软骨退行性变的临床应用价值。方法20例OA患者的患侧膝关节行T1ρ、T2 mapping序列扫描,以T2 mapping序列作为对比,测量髌骨、股骨软骨的T1ρ、T2值,与20例正常组比较;同时计算各部分软骨的T1ρ、T2值Z-score 的置信区间、Z-score的相关性(Pearson相关)及了解其分布情况。结果 OA组髌骨、股骨软骨T1ρ值分别为(50.01±4.23)ms、(53.80±4.38)ms,较正常组(45.12±0.92)ms、(45.42±0.84)ms高,两组间比较差异有统计学意义(t=5.077,P<0.001;t=8.446,P<0.001)。OA组与正常组软骨的T1ρ、T2值之间具有相关性[OA组相关系数为0.548±0.170,置信区间为(0.396,0.812),正常组相关系数为(0.455±0.163),置信区间为(0.246,0.789)]。结论 OA患者中关节软骨T1ρ值升高,且T1ρ与T2值两者的变化具有一定的相关性。T1ρ与T2 mapping能互补的诊断关节软骨早期退变,两技术可作为临床诊断的检查方法。

목적:탐토T1ρ대슬관절연골퇴행성변적림상응용개치。방법20례OA환자적환측슬관절행T1ρ、T2 mapping서렬소묘,이T2 mapping서렬작위대비,측량빈골、고골연골적T1ρ、T2치,여20례정상조비교;동시계산각부분연골적T1ρ、T2치Z-score 적치신구간、Z-score적상관성(Pearson상관)급료해기분포정황。결과 OA조빈골、고골연골T1ρ치분별위(50.01±4.23)ms、(53.80±4.38)ms,교정상조(45.12±0.92)ms、(45.42±0.84)ms고,량조간비교차이유통계학의의(t=5.077,P<0.001;t=8.446,P<0.001)。OA조여정상조연골적T1ρ、T2치지간구유상관성[OA조상관계수위0.548±0.170,치신구간위(0.396,0.812),정상조상관계수위(0.455±0.163),치신구간위(0.246,0.789)]。결론 OA환자중관절연골T1ρ치승고,차T1ρ여T2치량자적변화구유일정적상관성。T1ρ여T2 mapping능호보적진단관절연골조기퇴변,량기술가작위림상진단적검사방법。
Objective To determine the value of T1ρMR imaging assessment of knee cartilage degeneration in osteoarthritis (OA).Methods Twenty patients with OA of knees and twenty healthy volunteers underwent T1ρand T2 mapping MR imaging.The T1ρand T2 values of the patellar and femoral cartilage were calculated and compared.The spatial correlation of T1ρand T2 values was analyzed using Z-scores.The Pearson correlation coefficients of T1ρand T2 Z-scores were calculated in each subcompartment. Results The average T1ρ values of patellar(50.01±4.23)ms and femoral(53.80±4.38)ms cartilage in OA were significantly (P<0.001)higher than that in healthy adults(45.12±0.92)ms,(45.42±0.84)ms.The T1ρ and T2 Z-scores of overall cartilage correlated between healthy adults(0.455±0.163)and OA patients(0.548±0.170).Conclusion T1ρand T2 values of cartilage in OA are increased and may provide complementary information regarding early cartilage degeneration.
