目的:观察丰富环境刺激联合水疗对新生儿缺氧缺血性脑病(HIE)的早期干预效果。方法将50例中、重度 HIE患儿随机分为刺激组和对照组各25例。2组均给予常规治疗和护理,刺激组在患儿惊厥控制后,给予丰富环境刺激联合水疗的方法。比较2组体质量、神经行为评分(NBNA)及发育商(DQ)。结果2组治疗前体质量和NBNA评分比较,差异无统计学意义,刺激组在日龄15天和28天时体质量和NBNA评分均高于对照组。刺激组在3个月和6个月的DQ值均高于对照组。结论丰富环境刺激联合水疗能够降低 H IE患儿的伤残率,促进患儿运动功能和智能发育,有助于改善新生儿 HIE的预后,是一种简单、有效、无创、依从性好的早期干预措施。

목적:관찰봉부배경자격연합수료대신생인결양결혈성뇌병(HIE)적조기간예효과。방법장50례중、중도 HIE환인수궤분위자격조화대조조각25례。2조균급여상규치료화호리,자격조재환인량궐공제후,급여봉부배경자격연합수료적방법。비교2조체질량、신경행위평분(NBNA)급발육상(DQ)。결과2조치료전체질량화NBNA평분비교,차이무통계학의의,자격조재일령15천화28천시체질량화NBNA평분균고우대조조。자격조재3개월화6개월적DQ치균고우대조조。결론봉부배경자격연합수료능구강저 H IE환인적상잔솔,촉진환인운동공능화지능발육,유조우개선신생인 HIE적예후,시일충간단、유효、무창、의종성호적조기간예조시。
Objective To observe the effects of enriched environment stimulation combined with hydrotherapy on neonates with hypoxic ischemic encephalopathy (HIE) . Methods Fifty neonates with HIE were randomly divided into control group(n=25)and stimulation group(n=25) .The control group was given routine nursing and seizure control .The stimulation group was given enriched environment and hydrotherapy at the base of control group . Body weight , neonatal behavioral neurological assessment (NBNA ) and developmental quotient (DQ) of 2 groups were compared .Results Before treatment ,NBNA score and weight in 2 groups had no significant differences .At the 15th and 28th day ,the weight and NBNA score in the stimulation group were higher than those in the control group .At the 3rd and 6th month ,the DQ in the stimulation group were higher than that in the control group .Conclusion Enriched environment stimulation combined with hydrotherapy can reduce the disability rate of children with HIE ,promote the motor functions and intellectual development ,be help to improve the prognosis of neonatal HIE .It is a simple ,effective , noninvasive ,good compliance of early intervention measures for neonates with HIE .
