目的：观察胃转流术对2型糖尿病的降糖效果并探讨其降糖机制。方法回顾性分析120例行胃转流术的2型糖尿病患者的临床资料，分析手术前后口服糖耐量、空腹血糖、体重指数、胰岛素抵抗指数、c肽、胰岛素刺激释放试验、饮食量的变化。结果术后1个月患BMI (体重指数)无明显变化，口服糖耐量及C肽、胰岛素刺激释放试验均较术前显著好转(P<0.05)，糖化血红蛋白有明显下降趋，糖尿病症状获得明显缓解。空腹胰岛素无明显的统计学意义(P>0.05)。术后随访，77例血糖控制在正常范围，不需药物控制，31例明显好转，血糖值<1/2(术前血糖值一治愈标准值)。术后个6月糖尿病的治愈率为90％(108/120)。结论胃转流手术是2型糖尿病一种有效的治疗方法。
목적：관찰위전류술대2형당뇨병적강당효과병탐토기강당궤제。방법회고성분석120례행위전류술적2형당뇨병환자적림상자료，분석수술전후구복당내량、공복혈당、체중지수、이도소저항지수、c태、이도소자격석방시험、음식량적변화。결과술후1개월환BMI (체중지수)무명현변화，구복당내량급C태、이도소자격석방시험균교술전현저호전(P<0.05)，당화혈홍단백유명현하강추，당뇨병증상획득명현완해。공복이도소무명현적통계학의의(P>0.05)。술후수방，77례혈당공제재정상범위，불수약물공제，31례명현호전，혈당치<1/2(술전혈당치일치유표준치)。술후개6월당뇨병적치유솔위90％(108/120)。결론위전류수술시2형당뇨병일충유효적치료방법。
Objective :To explore the hypoglycemic effects and mechanism on patients with Type 2 diabetes by gastric by-pass operation. Method: We retrospectively analysed the clinical data of 120 patients with type 2 diabetes underwent gastric by-pass operation in our hospital.The clinical parameters such as oral glucose tolerance test (OGTT), the fasting blood glucose level, the body mass index (BMI), the insulin resistance index, C- peptide releasing test (CPRT) and insulin releasing test (IRT), Changes in food intake were evaluated before and after surgery. Results: At one month after operation,there were no significant changes in BMI(P>0.05),but glycosylated hemoglobin,OGTY,CPRT and IRT were significantly ameliorated(P<0.05),and the diabetic symptoms were markedly relieved. While the fasting insulin levels had no significant changes (P>0.05). Fol owed up for more than 3 month after the operation, the blood glucose level of 77 patients were normal,there was no need of drugs,31 patients were significantly ameliorated,and their blood glucose levels<1/2.6 months after surgery, the cure rate of diabetes was 90% (108/120)Conclusion: Gastric bypass operation can effectively treat the type 2 diabetes.
