目的:探讨α-硫辛酸对 IV 期糖尿病肾病(DN)患者的肾脏保护作用。方法将66例 IV 期 DN 患者随机分为常规治疗组(常规组)及α-硫辛酸组(观察组),观察2组治疗前后血压、血糖、血脂、血清超氧化物歧化酶(SOD)、丙二醛(MDA)、尿微量白蛋白排泄率(UAER)、尿β2-微球蛋白(β2-MG)、肌酐(SCr)、尿素氮(BUN)的变化。另选取健康体检者20例作为健康对照组。结果常规组及观察组治疗前后血压、血糖、血脂比较差异无统计学意义( P >0.05),而 FPG、2hPG、HbA1c高于健康对照组( P <0.05);常规组及观察组治疗前 MDA、UAER、β2-MG 均高于健康对照组,SOD 低于健康对照组( P <0.05)。治疗后2组 UAER、β2-MG 均下降,观察组较常规组下降更明显( P均<0.05);观察组治疗后 MDA 下降、SOD 升高且优于常规组( P <0.05)。结论α-硫辛酸能够有效控制 IV 期 DN的蛋白尿,延缓 DN 进展,对肾脏起到保护作用。

목적:탐토α-류신산대 IV 기당뇨병신병(DN)환자적신장보호작용。방법장66례 IV 기 DN 환자수궤분위상규치료조(상규조)급α-류신산조(관찰조),관찰2조치료전후혈압、혈당、혈지、혈청초양화물기화매(SOD)、병이철(MDA)、뇨미량백단백배설솔(UAER)、뇨β2-미구단백(β2-MG)、기항(SCr)、뇨소담(BUN)적변화。령선취건강체검자20례작위건강대조조。결과상규조급관찰조치료전후혈압、혈당、혈지비교차이무통계학의의( P >0.05),이 FPG、2hPG、HbA1c고우건강대조조( P <0.05);상규조급관찰조치료전 MDA、UAER、β2-MG 균고우건강대조조,SOD 저우건강대조조( P <0.05)。치료후2조 UAER、β2-MG 균하강,관찰조교상규조하강경명현( P균<0.05);관찰조치료후 MDA 하강、SOD 승고차우우상규조( P <0.05)。결론α-류신산능구유효공제 IV 기 DN적단백뇨,연완 DN 진전,대신장기도보호작용。
Objective To investigate the protective effect of α-lipoic acid on patients with stage IV diabetic nephropathy (DN).Methods 66 patients with stage IV DN were randomly divided into conventional treatment group (conventional group) andα-lipoic acid group (observation group), 2 groups'after treatment blood pressure, blood sugar, blood lipids, serum superoxide dis-mutase (SOD), malondialdehyde (MDA), urinary albumin excretion rate (UAER), urinary β2-microglobulin (β2-MG), creati-nine (SCr), blood urea nitrogen (BUN) changes was observed.Another 20 cases of healthy people selected as healthy control group.Results Conventional group and observation group before and after treatment, differences of blood pressure, blood glucose, blood lipids were not statistically significant ( P >0.05), while the FPG, 2hPG, HbA1c higher than the healthy control group (P <0.05); before treatment, conventional group and the observation group's MDA, UAER, β2-MG were higher than the healthy control group, SOD lower than the control group ( P <0.05).After treatment, the two groups'UAER, β2-MG decreased, observa-tion group decreased more significantly than the conventional group ( P <0.05); after treatment, in the observation group, MDA decreased and SOD increased, and better than the conventional group ( P <0.05).Conclusion α-lipoic acid can effectively con-trol proteinuria of stage IV DN, slow DN progress, play a protective role for the kidneys.
