目的：探讨组织速度显像（Tissue velocity imaging，TVI）结合实时三维超声心动图（real time three-dimension echocardiography，RT-3DE）在右心衰竭患者右室功能评价中的应用价值。方法应用 TVI 结合 RT-3DE技术，对新疆医科大学附属中医医院超声诊断为右室增大患者47例与正常对照组50例进行右室功能分析。根据有无右心衰竭临床表现将右室增大患者分为 A 组（右心衰竭）20例、B 组（无右心衰竭）27例，分别测其心尖四腔心切面右室游离壁三尖瓣瓣环的组织速度 Vs、Ve 及 Ve/Va 值，实时三维测 RVEDV、RVESV、RVSV 及 RVEF 值。结果右室增大 A 组、B 组患者 RVEDV、RVESV 均高于正常对照组（P ＜0．05），RVSV 和 RVEF 及 Vs、Ve、Ve/Va 值均低于正常对照组（P ＜0．05）。结论TVI 结合实时三维超声可较敏感的评估右室地舒缩功能。
목적：탐토조직속도현상（Tissue velocity imaging，TVI）결합실시삼유초성심동도（real time three-dimension echocardiography，RT-3DE）재우심쇠갈환자우실공능평개중적응용개치。방법응용 TVI 결합 RT-3DE기술，대신강의과대학부속중의의원초성진단위우실증대환자47례여정상대조조50례진행우실공능분석。근거유무우심쇠갈림상표현장우실증대환자분위 A 조（우심쇠갈）20례、B 조（무우심쇠갈）27례，분별측기심첨사강심절면우실유리벽삼첨판판배적조직속도 Vs、Ve 급 Ve/Va 치，실시삼유측 RVEDV、RVESV、RVSV 급 RVEF 치。결과우실증대 A 조、B 조환자 RVEDV、RVESV 균고우정상대조조（P ＜0．05），RVSV 화 RVEF 급 Vs、Ve、Ve/Va 치균저우정상대조조（P ＜0．05）。결론TVI 결합실시삼유초성가교민감적평고우실지서축공능。
Objective We evaluated the right ventricular function with tissue velocity imaging (TVI)and real time three-dimension echocardiography (RT-3DE).Methods 50 healthy control patients and 47 patients with right ven-tricular enlargement in our hospital were divided into group A (20 cases)and B (27 cases)according to the pres-ence or absence of right heart failure.All patients obtained both tissue velocity imaging of ultrasound examination and real time three-dimension echocardiography.The data were obtained from the velocity of right ventricular free wall′s three tricuspid valve ring on apical four chamber view,including Vs,Ve,Ve/Va ratios,EDV,ESV,SV and EF.Results ESV and EDV of patients in group A and B were higher than those of the control group (P <0.05).SV,EF,Vs,Ve and Ve/Va ratios of patients in group A and B were lower than those of the control group (P <0.05).Conclusion TVI combined with RT-3DE could assess more sensitively the right ventricular systolic and diastolic function.
