目的:分析预防接种后死亡病例的特征,评价预防接种的安全性,为安全预防接种提供科学依据。方法通过中国免疫规划监测信息管理系统收集河南省2010年1月1日-2011年12月31日报道的预防接种后死亡个案数据,采用描述性方法对相关指标进行流行病学分析。结果报道的7例预防接种后死亡病例AEFI分类均为偶合症,3例病例临床诊断为婴儿猝死综合征。7例预防接种后死亡病例接种后发病时间中位数为0 d,接种后死亡时间中位数为1 d。7例死亡病例接种的10剂次疫苗有8剂次为首次接种。引起预防接种后死亡的可疑疫苗分别为百白破(无细胞)、脊灰(减毒猴肾)、Hib、流感(裂解)、百白破、麻疹、乙脑(减毒)和卡介苗。结论偶合症是预防接种后死亡常见的类型,儿童接种尤其是疫苗首剂次后在一段相当长的时间范围内应密切关注儿童身体状况。建立完善预防接种后伤害救济机制是处理预防接种后死亡的关键措施。

Objective To evaluate the safety of vaccine by analyzing the death cases after vaccination and provide a scientific basis for the safety of vaccination.Methods The death cases after vaccination were collected through China information system for NIP surveillance and reported from June 1,2010 to Dec 31,2011.The descriptive methodology is used in this study.Results 7 death cases were reported in the NIP surveillance system and were all classified as coupling disease,and three cases were diagnosed as sudden infant death syndrome.The median time of episode and death after inoculation was 0 day and one day, respectively.Eight of ten vaccinations were the first time vaccination vaccine in the seven cases.The suspicious vaccines which caused the death after vaccination were DPT (acellular),polio(attenuated monkey kidney),Hib,influenza (cracking),DPT ,measles,JE(attenuated) and BCG. Conclusion Coincidental event is common type in death after vaccination.The children,especially after the first doses,should be paid close attention in quite a long time after vaccination.Establish and improve the vaccination injury relief mechanism is the key measure for dealing with the death cases following immunization.
