目的:探讨棘突椎板复合体原位回植椎管成形术在椎管肿瘤术中的意义。方法分析总结2010年1月至2013年6月间共18例椎管内肿瘤患者术中采用铣刀卸下椎板,处理完椎管内肿瘤后再将棘突椎板复合体原位回植,予钛条钛钉固定,恢复椎管正常解剖结构。结果所有病例未出现因铣刀卸下椎板,棘突椎板复合体原位回植固定而导致硬脊膜破损、脊髓和脊神经根损伤等并发症。随访3~18个月,未见回植组织移位、塌陷、畸形愈合及椎管狭窄症。结论椎管内肿瘤术中棘突椎板复合体原位回植椎管成形,钛条钛钉固定的方法简便、实用,有利于恢复脊柱的正常解剖结构及稳定性。

목적:탐토극돌추판복합체원위회식추관성형술재추관종류술중적의의。방법분석총결2010년1월지2013년6월간공18례추관내종류환자술중채용선도사하추판,처리완추관내종류후재장극돌추판복합체원위회식,여태조태정고정,회복추관정상해부결구。결과소유병례미출현인선도사하추판,극돌추판복합체원위회식고정이도치경척막파손、척수화척신경근손상등병발증。수방3~18개월,미견회식조직이위、탑함、기형유합급추관협착증。결론추관내종류술중극돌추판복합체원위회식추관성형,태조태정고정적방법간편、실용,유리우회복척주적정상해부결구급은정성。
Objective To investigate the spinous process and vertebral plate complex in situ replantation laminoplasty in intraspinal tumor clinical signiifcance. Methods From January to 2013 of 2010 are analyzed in June a total of 18patients with intraspinal tumor treated by cutter removed lamina, after treatment of intraspinal tumors after thespinous process and vertebral plate complex in situ replantation with titanium, titanium screw ifxation, restore thenormal anatomic structure of spinal canal. Results All patients does not appear due to milling cutter removed lamina,spinous process and vertebral plate complex in situ replantation ifxed and lead to breakage, epidural spinal cord and spinal nerve root injury and other complications. Patients were followed up for 3~18 months, no implant tissue displacement, collapse, malunion and spinal stenosis. Conclusions Intraspinal tumors spinous process and vertebral plate complex in situ replantation laminoplasty, titanium titanium screw ifxation method is simple, practical, is conducive to the normal anatomical structure and restoration of spinal stability.
