目的：讨论高血压脑出血术后发生再次出血的原因及防治措施。方法：回顾性分析近5年来笔者所在医院诊治的32例高血压脑出血患者术后再次出血的临床资料。结果：17例行再次血肿清除术，12例保守治疗，余3例患者放弃治疗自动出院。出院时生活能自理6例；遗留较重的后遗症：偏瘫、失语、生活不能自理17例；植物生存状态3例；死亡3例。结论：高血压脑出血术后再次出血的主要因素为术中止血不良，术后血压控制较差且波动幅度较大所致。术中妥善止血及术后保持血压平稳是避免和减少术后再出血的关键因素。

목적：토론고혈압뇌출혈술후발생재차출혈적원인급방치조시。방법：회고성분석근5년래필자소재의원진치적32례고혈압뇌출혈환자술후재차출혈적림상자료。결과：17례행재차혈종청제술，12례보수치료，여3례환자방기치료자동출원。출원시생활능자리6례；유류교중적후유증：편탄、실어、생활불능자리17례；식물생존상태3례；사망3례。결론：고혈압뇌출혈술후재차출혈적주요인소위술중지혈불량，술후혈압공제교차차파동폭도교대소치。술중타선지혈급술후보지혈압평은시피면화감소술후재출혈적관건인소。
Objective:To explore the reasons and preventive strategies of re-hemorrhage in patients with hypertensive intracerebral hemorrhage(ICH). Method:The data of 32 cases hypertensive ICH patients with re-hemorrhage after operation within recent 5 years were analyzed retrospectively.Result:The second operation with hematoma evacuation was performed in 17 cases;12 cases received conservative therapy,and the remaining 3 cases were discharged voluntarily.At discharge,6 cases could live independently,17 cases had severe sequelae including hemiplegia,aphasia and dependent living.Vegetative state was in 3 cases, and died in 3 cases.Conclusion:The major reasons of re-hemorrhage in patients with hypertensive ICH include intraoperative poor hemostasis and postoperative large blood pressure fluctuation.The good intraoperative hemostasis and postoperative stable blood pressure control are the most important factors to reduce the occurrence of re-hemorrhage.
