目的：探讨应用负压封闭引流(vacuum sealing drainage，VSD)技术结合医德保持续负压灌洗、引流治疗术区脂肪液化的临床疗效。方法：选取笔者所在医院2010年12月-2013年1月收治的36例四肢及骨盆术后发生脂肪液化的患者，对所有患者均行脂肪液化区拆除缝线、清创，采用VSD结合医德保持续负压灌洗、引流1~2周，5~7 d更换1次新的VSD敷料，待创面清洁，肉芽新鲜后再行伤口缝合术。结果：34例患者经5~7 d VSD封闭引流，脂肪液化区肉芽组织生长良好，缝合皮肤后均愈合；2例患者因VSD堵管，再一次清创后继续VSD引流7 d，肉芽组织新鲜平整，缝合皮肤后均基本愈合。结论：VSD结合医德保负压灌洗、引流能较好地清除脂肪液化区渗液，促进肉芽生长，二期缝合愈合率高，是治疗术区脂肪液化较理想的方法之一，值得推广。
목적：탐토응용부압봉폐인류(vacuum sealing drainage，VSD)기술결합의덕보지속부압관세、인류치료술구지방액화적림상료효。방법：선취필자소재의원2010년12월-2013년1월수치적36례사지급골분술후발생지방액화적환자，대소유환자균행지방액화구탁제봉선、청창，채용VSD결합의덕보지속부압관세、인류1~2주，5~7 d경환1차신적VSD부료，대창면청길，육아신선후재행상구봉합술。결과：34례환자경5~7 d VSD봉폐인류，지방액화구육아조직생장량호，봉합피부후균유합；2례환자인VSD도관，재일차청창후계속VSD인류7 d，육아조직신선평정，봉합피부후균기본유합。결론：VSD결합의덕보부압관세、인류능교호지청제지방액화구삼액，촉진육아생장，이기봉합유합솔고，시치료술구지방액화교이상적방법지일，치득추엄。
Objective:To discuss the clinical efficacy of vacuum sealing drainage(VSD) with continuous negative pressure lavaging and drainage combined with disinfectant in the treatment of operation area with fat liquefaction.Method:Select 36 patients with fat liquefaction in author’s department from December 2010 to January 2013,which all were treated through surgical debridement,and given VSD with continuous negative pressure lavaging and drainage for 1-2 weeks,VSD dressing was replaced every 5-7 days.The growth of granulation tissue was observed,and then skin was sutured.Result:The 34 patients,granulation tissue in wound grew well which were given VSD closed drainage after 5-7 days;2 patients were debrided once again because of plugging in VSD and continued to use VSD drainage for 7 days,fresh granulation tissue grown,wounds were healed after sutured.Conclusion:The application of VSD with continuous negative pressure lavaging and drainage combined with disinfectant can clear away seepage of fat liquefaction,promote the growth of granulation tissue,enhance the rate of healing,and which can be considered one of the effective methods for the treatment of operation area with fat liquefaction and is worthy to be carry out.
