目的:比较中医传统正骨手法结合微创经皮钢板内固定(MIPPO)技术锁定加压钢板(LCP)内固定和MIPPO技术、LCP内固定治疗胫骨远端骨折的临床疗效。方法:将我院2006年1月-2013年9月收治的胫骨远端骨折患者68例随机分为研究组(n=36)和对照组(n=30),分别采用中医传统正骨手法结合 MIPPO 技术 LCP内固定和单纯 MIPPO 技术 LCP 内固定,比较两组疗效。结果:全部病人均获随访3-22个月,平均7.5个月,按Johner-Wruhs 评分标准,研究组优良率为94.4%,明显高于对照组的87.5%(P﹤0.05);研究组在手术时间、透视次数、骨痂形成时间、骨折愈合时间和并发症的发生率方面均明显优于对照组(P﹤0.05)。结论:中医正骨手法结合MIPPO 技术 LCP 内固定治疗胫骨远端骨折的术中透视少、骨折愈合快、并发症少,是治疗胫骨远端骨折的有效方法。

목적:비교중의전통정골수법결합미창경피강판내고정(MIPPO)기술쇄정가압강판(LCP)내고정화MIPPO기술、LCP내고정치료경골원단골절적림상료효。방법:장아원2006년1월-2013년9월수치적경골원단골절환자68례수궤분위연구조(n=36)화대조조(n=30),분별채용중의전통정골수법결합 MIPPO 기술 LCP내고정화단순 MIPPO 기술 LCP 내고정,비교량조료효。결과:전부병인균획수방3-22개월,평균7.5개월,안Johner-Wruhs 평분표준,연구조우량솔위94.4%,명현고우대조조적87.5%(P﹤0.05);연구조재수술시간、투시차수、골가형성시간、골절유합시간화병발증적발생솔방면균명현우우대조조(P﹤0.05)。결론:중의정골수법결합MIPPO 기술 LCP 내고정치료경골원단골절적술중투시소、골절유합쾌、병발증소,시치료경골원단골절적유효방법。
Objective:To compare the clinical efficacy for patients with distal tibial fracture treated by traditional bone setting manipulation and minimally invasive percutaneous plate osteosynthesis (MIPPO)technology of LCP internal fixation. Methods: 68 patients with distal tibial fracture were randomly divided into research group (n=36)and control group (n=30) from Jan 2006 to Sept 2013, and treated by traditional bone setting manipulation combined with MIPPO technology of LCP internal fixation and simply by MIPPO LCP internal fixation respectively. The clinical efficacy was compared between the two groups. Results: All the cases were followed up for 3 - 22 months and the average was 7.5 months,according to the score standard evaluation of Johner-Wruhs,the excellent and good ratein research group was significantly higher than that in the control group (94.4% & 87.5%,P < 0.05). The operation time , fluoroscopy time , callus formation time , fracture healing time and the incidence of complications in the research group were significantly better than those in the control group (all P < 0.05). Conclusions: Using of traditional bone setting manipulation and MIPPO technology of LCP internal fixation in treatment of distal tibial fractures is an effective method because of reducing operation times of radiology and complications ,shorting the healing time.
