微卫星不稳定（MSI）是指 DNA 序列中简单重复序列的碱基长度发生变化，其发生主要由于错配修复基因失活引起，是结直肠癌发病的重要分子生物学途径之一。MSI大概占所有大肠癌的15%，其中3%与遗传性非息肉病性结直肠癌有关，其余12%见于散发性结直肠癌。MSI的大肠癌有以下特征：发生部位大多位于右半结肠；分化程度多为低分化，同时伴有淋巴细胞浸润；组织类型多为黏液型或者印戒细胞类型。MSI的发现增加了对大肠癌多样性的认识，并且对于患者的相关治疗及预后有一定的启示。

Microsatellite instability (MSI) means simple repetitive sequence of bases in DNA is changed. It is mainly caused by the mismatch repair gene inactivation. It is one of the important biological paths leading to colorectal tumorigenesis. MSI appears in around 15% of colorectal cancer, 3% of which is associated with hereditary nonpolyposis colorectal cancer, the remaining 12% can be found in sporadic colorectal cancer. Colorectal cancer with MSI phenotype has the following features: occurs mostly in the right colon, low differentiation, lymphocyte infiltration, mucous type or signet ring cell tissue types. The found of MSI increased awareness of the diversity of colorectal cancer. Meanwhile it has certain enlightenment for the patients' treatment.
