目的:探讨磁共振成像(MRI)对血行播散性肺结核并发结核性脑膜炎的临床诊断价值。方法对血行播散性肺结核50例进行了MRI平扫及增强扫描检查,对其中18例并发结核性脑膜炎的MRI特点进行分析。结果血行播散性肺结核合并结核性脑膜炎MRI影像表现为:头部平扫及增强扫描T1WI绝大部分病例显示程度不同的脑膜增厚,有明显强化;部分病例合并脑结核瘤;脑实质内粟粒型结核病灶增强扫描呈多发小结节状明显强化。结论 MRI检查尤其是增强扫描可作为血行播散性肺结核并发结核性脑膜炎重要的影像学检查方法,为其临床诊断和疗效观察提供可靠依据。

목적:탐토자공진성상(MRI)대혈행파산성폐결핵병발결핵성뇌막염적림상진단개치。방법대혈행파산성폐결핵50례진행료MRI평소급증강소묘검사,대기중18례병발결핵성뇌막염적MRI특점진행분석。결과혈행파산성폐결핵합병결핵성뇌막염MRI영상표현위:두부평소급증강소묘T1WI절대부분병례현시정도불동적뇌막증후,유명현강화;부분병례합병뇌결핵류;뇌실질내속립형결핵병조증강소묘정다발소결절상명현강화。결론 MRI검사우기시증강소묘가작위혈행파산성폐결핵병발결핵성뇌막염중요적영상학검사방법,위기림상진단화료효관찰제공가고의거。
Objective To evaluate the significance of MRI findings in diagnosis magnetic resonance imaging (MRI)in diagnosis of hematogenous disseminated pulmonary tuberculosis complicated with tuberculous meningitis. Methods Plain and enhanced MRI were performed in 50 cases of hematogenous disseminated pulmonary tuberculosis complicated with tuberculous meningitis,the MRI features of 18 cases with tuberculous meningitis were analyzed. Results Hematogenous disseminated pulmonary tuberculosis complicated with tuberculous meningitis MRI images showed the plain and enhanced scanning T1WI most cases showed different degrees of meningeal thickening,enhanced obviously. Some patients combined with cerebral tuberculoma,intraparenchymal miliarytuberculosis lesions enhanced scan showed multiple small nodular enhancement. Conclusion MRI examination especially enhanced scan can be used as an mportant imaging method for hehema hematogenous disseminated pulmonary tuberculosis complicated with tuberculous meningitis important imaging methods,and can provide a reliable basis for clinical diagnosis and curative effect observation.
