目的：探讨脑白质疏松（LA ）与脑大血管疾病（LVD ）在流行病学危险因素上的异同。方法回顾性纳入307名脑梗死或TIA的患者，通过头颅MRI评估LA ，血管DSA评估LVD ，同时登记患者血管危险因素。根据头颅MRI和DSA评估结果将患者分成三组：LA组，LVD组及对照组。应用单因素和多因素分析探讨各个危险因素在三组之间的差异。结果性别、年龄、高血压、糖尿病、同型半胱氨酸（Hcy ）、总胆固醇（TC ）、低密度脂蛋白（LDL ）、吸烟史、他汀类用药史在三组患者中分布不全相同（ P＜0.05）。年龄、高血压、Hcy与LA呈正相关（OR＝1.12～5.66，P＜0.01）；年龄、男性、高血压、糖尿病与LVD呈正相关（OR＝1.08～4.19，P＜0.01）。结论 LA与LVD在病因学上既有相同的危险因素，同时也存在着显著的差异。
목적：탐토뇌백질소송（LA ）여뇌대혈관질병（LVD ）재류행병학위험인소상적이동。방법회고성납입307명뇌경사혹TIA적환자，통과두로MRI평고LA ，혈관DSA평고LVD ，동시등기환자혈관위험인소。근거두로MRI화DSA평고결과장환자분성삼조：LA조，LVD조급대조조。응용단인소화다인소분석탐토각개위험인소재삼조지간적차이。결과성별、년령、고혈압、당뇨병、동형반광안산（Hcy ）、총담고순（TC ）、저밀도지단백（LDL ）、흡연사、타정류용약사재삼조환자중분포불전상동（ P＜0.05）。년령、고혈압、Hcy여LA정정상관（OR＝1.12～5.66，P＜0.01）；년령、남성、고혈압、당뇨병여LVD정정상관（OR＝1.08～4.19，P＜0.01）。결론 LA여LVD재병인학상기유상동적위험인소，동시야존재착현저적차이。
Objective To investigate the similarities and differences of risk factors between leukoaraiosis (LA) and cerebral large ves-sel disease(LVD).Methods A total of 307 patients with cerebral infarction or TIA were included.All patients underwent cranial MRI examinations to assess LA and underwent DSA examinations to assess LVD.The cerebral vascular risk factors were collected in the meantime.According to the results ,all patients were divided into three groups ,LA group ,LVD group and control group.Then a u-nivariate analysis and a binary Logistic regression analysis were used to investigate the similarities and differences of risk factors among the three groups.Results The distribution of sex ,age ,hypertension ,diabetes mellitus ,Hcy ,TC ,LDL ,smoking and statin usage in three groups were different (P<0.05).Age [OR=1.15 ,95% CI(1.10~1.21)] ,hypertension [OR=5.66 ,95% CI(2.38~13.47)] ,Hcy [OR=1.12 ,95% CI(1.05~1.20)] were the independent risk factors of LA ;age [OR=1.08 ,95% CI(1.04~1.21)] ,man [OR=3.15 ,95% CI(1.59~6.22)] ,hypertension [OR=2.09 ,95% CI(1.08~4.04)] ,diabetes mellitus [OR=4.19 ,95% CI(1.76~9.99)] were the independent risk factors of LVD (all P<0.05).Conclusions LA and LVD have some same risk factors ,and there were significant differences between the two groups.
