目的:探讨三维计算机断层扫描血管造影(3D-CTA)在颅内动脉瘤诊断和早期手术中的临床应用。方法纳入2008-10-2012-10我院收治的颅内动脉瘤患者40例,均行3D-CTA和DSA检查获取CT三维图像,对颅内动脉显示情况、动脉瘤检出个数及其动脉瘤颈与载瘤血管的关系进行回顾性分析。结果3D-C T A图像清晰可见颅内动脉主干及分支,与DSA图像结果比较无显著性差异(P>0.05)。动脉瘤最大病灶者直径16.5 mm ,最小病灶者直径1.1 mm。40例患者中共检查出动脉瘤48个,其中33例为单发,7例为多发。3D-CTA方法检查出动脉瘤42个,DSA方法检查出动脉瘤45个,二者比较差异无统计学意义(P>0.05)。3D-CTA更能清晰显示载瘤血管与动脉瘤颈形态的三维立体结构,同时,清晰显示周围颅底骨质与动脉瘤体的三维立体关系。结论3D-T C A可有效应用于临床诊断颅内动脉瘤,准确有效地评价颅内动脉瘤形态,作为影像学检查的常规方法,为临床治疗提供参考依据。

목적:탐토삼유계산궤단층소묘혈관조영(3D-CTA)재로내동맥류진단화조기수술중적림상응용。방법납입2008-10-2012-10아원수치적로내동맥류환자40례,균행3D-CTA화DSA검사획취CT삼유도상,대로내동맥현시정황、동맥류검출개수급기동맥류경여재류혈관적관계진행회고성분석。결과3D-C T A도상청석가견로내동맥주간급분지,여DSA도상결과비교무현저성차이(P>0.05)。동맥류최대병조자직경16.5 mm ,최소병조자직경1.1 mm。40례환자중공검사출동맥류48개,기중33례위단발,7례위다발。3D-CTA방법검사출동맥류42개,DSA방법검사출동맥류45개,이자비교차이무통계학의의(P>0.05)。3D-CTA경능청석현시재류혈관여동맥류경형태적삼유입체결구,동시,청석현시주위로저골질여동맥류체적삼유입체관계。결론3D-T C A가유효응용우림상진단로내동맥류,준학유효지평개로내동맥류형태,작위영상학검사적상규방법,위림상치료제공삼고의거。
Objective To investigate the three-dimensional computed tomography angiography (3D-CTA) in the diagnosis of intracranial aneurysms and early surgery on clinical application.Methods Forty patients with aneurysms admitted to our hospital from October 2008 to October 2012 were chosen as subjects.All patients underwent 3D-CTA and DSA to obtain three-dimensional CT images. The situation of arterial aneurysms displayed ,the number of aneurysms detected ,the relationship be-tween aneurysm neck and vascular aneurysm were retrospectively analyzed.Results 3D-CTA image clearly displayed intracrani-al artery and its branches ,and showed no significant difference from DSA images results (P>0.05). The maximum diameter of aneurysm was 16.5 mm ,diameter of the smallest lesions was 1.1 mm.Forty-eight aneurysms were checked out from 40 pa-tients ,of which 33 cases were single ,seven cases were multiple. 3D-CTA checked out 42 aneurysms ,DSA checked out 45 an-eurysm ,comparison the two methods showed no significant difference (P>0.05). 3D-CTA more clearly showed the three-di-mensional structure of aneurysm and aneurysm neck vessels ,and simultaneously clearly showed the relationship of three-dimen-sional structures between the skull base and the aneurysms.Conclusion 3D-TCA can be effectively used in clinical diagnosis of intracranial aneurysms ,accurately and effectively evaluates the morphology of intracranial aneurysms ,and can be used as con-ventional imaging methods to provide reference for clinical treatment.
