目的探讨超声HD-flow显像技术和MRI在产前诊断胎盘植入中的价值,并进行比较。方法选择20例可疑胎盘植入的患者行超声HD-flow显像技术和MRI检查,将手术和(或)病理结果作为金标准,分析并比较两种方法的诊断价值。结果经超声HD-flow显像诊断产前胎盘植入的敏感性﹑特异性﹑阳性和阴性预测值分别为77.8%(7/9)、72.7%(8/11),70.0%(7/10),80.0%(8/10),MRI敏感性﹑特异性﹑阳性和阴性预测值分别为77.8%(7/9)、45.4%(5/11),53.8%(7/13),71.4%(5/7)。结论超声 HD-flow 显像技术对胎盘植入的诊断敏感性和特异性均较高,在超声HD-flow显像提示可疑植入的情况下应进一步行MRI检查,为临床提供较为准确的诊断依据。

Objective To discuss and compare the value of ultrasonic HD-flow imaging and magnetic resonance imaging ( MRI) in the prenatal diagnosis of placenta increta. Methods 20 patients with suspected placenta increta underwent Ultrasonic HD-flow imaging and MRI test, the surgical and /or pathological results as the gold standard, analyze and compare the diagnostic value of two methods.Results The sensitivity of ultrasonic HD-flow imaging in diagnosis of placenta increta.was 77.8%( 7/9 ) and the specificity was 72.7%(8/11).The positive and negative predictive values were 70.0%(7/10) and 80.0%(8/10) re-spectively.The sensitivity of MRI in diagnosis of placenta increta was 77.8%( 7/9 ) and the specificity was 45.4%( 5/11 ).The positive and negative predictive values were 53.8%(7/13)and 71.4%(5/7) respectively.Conclusion Ultrasonic HD-flow ima-ging techniques in the diagnosis of placenta increta sensitivity and specificity were higher, Patients with ultrasonic findings suspi-cious for implant should refer to MRI, provide a more accurate clinical diagnosis.
