目的：了解慢性丙型肝炎（CHC）患者血清白介素-18（IL-18）与丙型肝炎病毒抗体（HCV-Ab）、病毒载量、丙氨酸氨基转移酶（ALT）的关系，探讨IL-18在CHC患者中的表达情况与临床意义。方法收集2012年12月至2013年4月广西医科大学第一附属医院门诊及病区就诊的47例CHC患者为CHC患者组，检测IL-18、病毒载量、HCV-Ab、ALT ，分析丙型肝炎病毒阳性、阴性，HCV-Ab阳性、阴性以及不同ALT 浓度患者IL-18的表达情况。另纳入同期高校体检学生20例为健康对照组，分析CHC患者与健康体检者IL-18的表达情况。结果 CHC患者组与健康对照组相比，IL-18浓度显著升高，差异有统计学意义（P＜0．01）。IL-18在 HCV-RNA阳性与阴性、HCV-Ab阳性与阴性间表达差异均无统计学意义（P＞0．05）；血清ALT水平与IL-18水平相关性分析，结果无明显相关性（r＝0．291，P＞0．05）。结论 IL-18与HCV慢性感染有关，但与HCV复制及其所致的肝损伤无明显相关性。
목적：료해만성병형간염（CHC）환자혈청백개소-18（IL-18）여병형간염병독항체（HCV-Ab）、병독재량、병안산안기전이매（ALT）적관계，탐토IL-18재CHC환자중적표체정황여림상의의。방법수집2012년12월지2013년4월엄서의과대학제일부속의원문진급병구취진적47례CHC환자위CHC환자조，검측IL-18、병독재량、HCV-Ab、ALT ，분석병형간염병독양성、음성，HCV-Ab양성、음성이급불동ALT 농도환자IL-18적표체정황。령납입동기고교체검학생20례위건강대조조，분석CHC환자여건강체검자IL-18적표체정황。결과 CHC환자조여건강대조조상비，IL-18농도현저승고，차이유통계학의의（P＜0．01）。IL-18재 HCV-RNA양성여음성、HCV-Ab양성여음성간표체차이균무통계학의의（P＞0．05）；혈청ALT수평여IL-18수평상관성분석，결과무명현상관성（r＝0．291，P＞0．05）。결론 IL-18여HCV만성감염유관，단여HCV복제급기소치적간손상무명현상관성。
Objective To explore the relationship of hepatitis C virus (HCV)-antiboby(Ab) ,viral load ,ala-nine aminotransferase (ALT) with interleukin-18 respectively ,and to discuss the expression level and clinical signifi-cance of IL-18 in patient with chronic hepatitis C (CHC) .Methods Blood serum and clinical datum of patients with CHC were collected ,who receiving treatment in outpatient and inpatient detpatment of this hospital from Dec .2012 to Apr .2013 .Serum concentration of IL-18 ,viral load ,HCV-Ab and ALT were detected .Expression level of IL-18 in patients of HCV-RNA(+ ) group ,HCV-RNA(-) group ,HCV-Ab(+ ) group and HCV-Ab(-) group and patients with different concentrations of ALT were compared .A total of 20 cases of healthy students in university were en-rolled as control group .Then the expression levels of IL-18 in patients with CHC and healthy subjects were com-pared .Results The concentration of IL-18 in CHC group increased significantly compared with control group (P<0 .01) .There was no statistically significant difference of concentration of IL-18 between HCV-RNA(+ ) group and HCV-RNA(-) group ,as well as between HCV-Ab(+ ) group and HCV-Ab(-) group (P>0 .05) .There was no significant correlation between level of IL-18 and ALT (r=0 .291 ,P>0 .05) .Conclusion IL-18 might be associated with chronic HCV infection ,but could be not significantly correlated with HCV replication and hepatic lesion caused by the latter .
