目的：探讨十二指肠损伤的早期诊断、合理处理及预后的影响因素。方法回顾性分析本院收治的21例十二指肠损伤患者的临床资料。结果手术治疗21例，治愈20例（95.2%，），死亡1例（4.8%）；术后并发症7例（35.0%），其中十二指肠瘘5例（25.0%）、胰瘘2例（10.0%）、腹腔脓肿3例（15.0%）、上消化道出血1例（5.0%）。术后并发症的危险因素分析表明患者确诊时间、术前休克、iss评分（损伤严重程度的评分标准）可能是预测患者预后的关键因素。结论早期作出诊断，及时纠正休克，选择合理的手术方式，可降低十二指肠损伤术后并发症的发生率。
목적：탐토십이지장손상적조기진단、합리처리급예후적영향인소。방법회고성분석본원수치적21례십이지장손상환자적림상자료。결과수술치료21례，치유20례（95.2%，），사망1례（4.8%）；술후병발증7례（35.0%），기중십이지장루5례（25.0%）、이루2례（10.0%）、복강농종3례（15.0%）、상소화도출혈1례（5.0%）。술후병발증적위험인소분석표명환자학진시간、술전휴극、iss평분（손상엄중정도적평분표준）가능시예측환자예후적관건인소。결론조기작출진단，급시규정휴극，선택합리적수술방식，가강저십이지장손상술후병발증적발생솔。
Objective To investigate the early diagnosis, treatment and prognostic factors of duodenal injury. Method The clinical data of 21 patients with duodenal injury were retrospectively analyzed. Result 21 cases were treated with operation, 20 cases were cured (95.2%), 1 cases died(4.8%);postoperative complications occurred in 7 cases(35.0%), including 5 cases of duodenal fistula (25.0%) , 2 cases of pancreatic fistula (10.0%), 3 cases of abdominal abscess (15.0%), 1 cases of upper gastrointestinal bleeding (5.0%).The time of diagnosis, preoperative shock, iss score may be a predictor of prognosis factors for patients. Conclusion Early diagnosis, timely and correct shock, reasonable operation can reduce the incidence of complications and mortality.
