目的：探讨前列地尔治疗糖尿病腔隙性脑梗死患者的临床疗效及对血糖、纤维蛋白原及甲襞微循环的影响。方法糖尿病腔隙性脑梗死患者95例随机分为治疗组48例和对照组47例。2组均以常规治疗为基础，对照组加用舒血宁治疗，观察组加用前列地尔治疗，治疗14 d后比较2组临床疗效、美国国立卫生院神经功能缺损评分（NIHSS）、血浆纤维蛋白原（Fg）、高敏C反应蛋白（hs-CRP）、空腹血糖（FPG）、糖化血红蛋白（HbA1c）及甲襞微循环变化。结果治疗组临床疗效明显高于对照组（91．6％ vs 78．8％， P ＜0．05）。2组NIHSS评分均改善，且治疗组优于对照组（ P ＜0．01， P ＜0．05）。2组Fg、hs-CRP、FPG及微循环指标均改善，且治疗组较对照组均明显好转（ P ＜0．05， P ＜0．01）。结论糖尿病腔隙性脑梗死患者应用前列地尔能够显著改善临床疗效，提高其神经功能，且无明显不良反应，值得推广应用。
목적：탐토전렬지이치료당뇨병강극성뇌경사환자적림상료효급대혈당、섬유단백원급갑벽미순배적영향。방법당뇨병강극성뇌경사환자95례수궤분위치료조48례화대조조47례。2조균이상규치료위기출，대조조가용서혈저치료，관찰조가용전렬지이치료，치료14 d후비교2조림상료효、미국국립위생원신경공능결손평분（NIHSS）、혈장섬유단백원（Fg）、고민C반응단백（hs-CRP）、공복혈당（FPG）、당화혈홍단백（HbA1c）급갑벽미순배변화。결과치료조림상료효명현고우대조조（91．6％ vs 78．8％， P ＜0．05）。2조NIHSS평분균개선，차치료조우우대조조（ P ＜0．01， P ＜0．05）。2조Fg、hs-CRP、FPG급미순배지표균개선，차치료조교대조조균명현호전（ P ＜0．05， P ＜0．01）。결론당뇨병강극성뇌경사환자응용전렬지이능구현저개선림상료효，제고기신경공능，차무명현불량반응，치득추엄응용。
To evaluate the clinical efficacy of alprostadil in treatment of diabetic patients with lacunar infarction and various indicators'changes .Methods Diabetic patients with lacunar infarction 95 cases were randomly divided into treatment group (48 cases ) and control group ( 47 cases ).2 groups are based on conventional therapy , the control group plus Shuxuening treatment, observation group were treated with alprostadil treatment , clinical efficacy compared between the two groups 14 d after treatment , the U.S.National Institutes of Health neurologic deficit score ( NIHSS ) , plasma fibrinogen (Fg), high-sensitivity C-reactive protein (hs-CRP), fasting plasma glucose (FPG), glycated hemoglobin (HbA1c) and nail fold microcirculation were observed .Results Clinical efficacy of the treatment group was significantly higher (91.6% vs. 78.8%, P <0.05).2 groups NIHSS scores were improved , and the treatment group better than the control group ( P <0.01, P <0.05).2 groups'Fg, hs-CRP, FPG and microcirculation indicators are improved , and the treatment group were significantly better than the control group ( P <0.05, P <0.01).Conclusion Alprostadil can significantly improve the clin-ical efficacy and improve neurological function in diabetic patients with lacunar infarction , and no significant adverse reactions were observed , it can be widely applied .
