目的:观察慢性心力衰竭患者心脏再同步化治疗( CRT)治疗前后心功能及血浆非对称性二甲基精氨酸( ADMA)和血浆N-端脑钠肽前体( NT-proBNP)水平的变化。方法21例慢性心力衰竭患者行CRT治疗,测定治疗前及治疗后3个月、6个月NYHA心功能分级、左室射血分数( LVEF)、左室舒张末内径( LVEDD)及血浆ADMA、NT-proBNP变化。结果21例慢性心力衰竭患者心衰症状改善,NYHA心功能分级提高,LVEF、LVEDD、NT-proBNP等心功能指标改善,血浆NT-proBNP和ADMA水平与治疗前比较明显下降,差异有统计学意义(P<0.05)。结论慢性心衰经CRT治疗后心功能改善,血浆NT-proBNP及ADMA水平降低。

목적:관찰만성심력쇠갈환자심장재동보화치료( CRT)치료전후심공능급혈장비대칭성이갑기정안산( ADMA)화혈장N-단뇌납태전체( NT-proBNP)수평적변화。방법21례만성심력쇠갈환자행CRT치료,측정치료전급치료후3개월、6개월NYHA심공능분급、좌실사혈분수( LVEF)、좌실서장말내경( LVEDD)급혈장ADMA、NT-proBNP변화。결과21례만성심력쇠갈환자심쇠증상개선,NYHA심공능분급제고,LVEF、LVEDD、NT-proBNP등심공능지표개선,혈장NT-proBNP화ADMA수평여치료전비교명현하강,차이유통계학의의(P<0.05)。결론만성심쇠경CRT치료후심공능개선,혈장NT-proBNP급ADMA수평강저。
Objective To investigate changes in heart function and the levels of asymmetric dimethylarginine ( ADMA) and N-terminal pro-brain natriuretic peptide(NT-proBNP) before and after cardiac resynchronization therapy (CRT) treatment in patients with chron-ic heart failure .Methods The levels of ADMA and NT-proBNP were measured in 21 heart failure patients before and three and six months after receiving CRT .The NYHA functional class ,left ventricular ejection fraction ,left ventricular end-diastolic dimension were also measured by echocardiography at the same time .Results Compared with pro-CRT,the NYHA functional class ,left ventricular e-jection fraction and the synchrony index improved ,while left ventricular end-diastolic dimension decreased after treatment of CRT .The plasma concentration of ADMA and NT-proBNP significantly decreased too . There was significant difference about them before and af-ter CRT treatment(P<0.05).Conclusions Cardiac function can be improved and the plasma levels of ADMA and NT-proBNP are decreased by treatment of CRT .
