目的：探讨肿瘤相关蛋白Galectin-3和P27与老年早期胃癌患者临床病理的关系，并分析其在微创手术临床预后方面的意义。方法选取58例该院消化内科在2010年7月至2013年10月收治的老年早期胃癌患者作为研究对象，所有患者均行内镜黏膜下剥离术（ ESD）。对每例患者术中所取肿瘤及周围正常组织进行Galectin-3和P27的免疫组化检测，并分析其表达量与临床病理的联系，最后分析两种蛋白与 ESD术后复发的关系。结果癌组织Galectin-3表达量明显高于正常组织，而P27明显低于正常组织（P＜0.05）；且分层分析示二者表达量与瘤灶大小和超声黏膜结构有关（P＜0.05）；进一步分析发现二者是否阳性表达与患者ESD术后复发密切相关（P＜0.05），Cox风险比例模型多因素分析进一步显示其影响患者预后（P＜0.05）。结论 Galectin-3和P27可能作为早期胃癌临床分子病理的评估指标，且对患者临床预后具有一定的预测意义。
목적：탐토종류상관단백Galectin-3화P27여노년조기위암환자림상병리적관계，병분석기재미창수술림상예후방면적의의。방법선취58례해원소화내과재2010년7월지2013년10월수치적노년조기위암환자작위연구대상，소유환자균행내경점막하박리술（ ESD）。대매례환자술중소취종류급주위정상조직진행Galectin-3화P27적면역조화검측，병분석기표체량여림상병리적련계，최후분석량충단백여 ESD술후복발적관계。결과암조직Galectin-3표체량명현고우정상조직，이P27명현저우정상조직（P＜0.05）；차분층분석시이자표체량여류조대소화초성점막결구유관（P＜0.05）；진일보분석발현이자시부양성표체여환자ESD술후복발밀절상관（P＜0.05），Cox풍험비례모형다인소분석진일보현시기영향환자예후（P＜0.05）。결론 Galectin-3화P27가능작위조기위암림상분자병리적평고지표，차대환자림상예후구유일정적예측의의。
Objective To investigate the relationship between tumor associated Galectin-3 and P27 protein and clinical pathology in elderly patients with early gastric cancer , and analyze their clinical prognostic significance in surgery with minimal invasion .Methods 58 elderly patients with early gastric cancer performed with endoscopic submucosal dissection ( ESD) surgery were selected as the research ob-ject.The immunohistochemical detection of Galectin-3 and P27 in intraoperative tumor tissues and corresponding normal tissues were per-formed on each patient , and to analyze the links between their expression and clinical pathological , and the relationship between the two kinds of protein and recurrence of cancer after ESD .Results The expression of Galectin-3 in tumor tissues was significantly higher than that of normal tissues, insteadly, the expression of P27 was obviously lower than that of normal group (P<0.05);the quantity expression of pro-tein was related to both the tumor size and ultrasonic mucosa structure (P<0.05);the two kinds of protein positive expression were closely related with postoperative recurrence of ESD (P<0.05), and the Cox risk proportion model with multi-factors showed their effects in the prognosis of patients(P<0.05).Conclusions Tumor associated protein Galectin-3 and P27 might be a clinical molecular pathological evalu-ation index for predicting clinical prognosis of early gastric cancer patients .
