目的：比较三维能量多普勒超声测量子宫内膜良、恶性病变患者的血流参数值；研究子宫内膜癌患者病理分期与血流参数的相关性。方法选择绝经后阴道流血患者55例，测定收缩期峰值流速（PSV）、搏动指数（PI）、阻力指数（RI）及血管形成指数（VI）。结果①子宫内膜癌组的超声血管参数PSV、PI、RI值均显著低于子宫内膜良性病变组，VI值显著高于子宫内膜良性病变组（P＜0.05）；②子宫内膜癌组的VI值在Ⅰ期与Ⅱ期、Ⅱ期与Ⅲ期分别比较有显著差异（ P＜0.05）；子宫内膜癌有、无淋巴结转移组间RI和VI值差异显著（ P＜0.05）。结论三维能量多普勒超声技术对子宫内膜良、恶性病变的诊断与鉴别诊断有重要临床价值，为临床术前评估子宫内膜癌的分期提供了一种相对准确的新手段。
목적：비교삼유능량다보륵초성측양자궁내막량、악성병변환자적혈류삼수치；연구자궁내막암환자병리분기여혈류삼수적상관성。방법선택절경후음도류혈환자55례，측정수축기봉치류속（PSV）、박동지수（PI）、조력지수（RI）급혈관형성지수（VI）。결과①자궁내막암조적초성혈관삼수PSV、PI、RI치균현저저우자궁내막량성병변조，VI치현저고우자궁내막량성병변조（P＜0.05）；②자궁내막암조적VI치재Ⅰ기여Ⅱ기、Ⅱ기여Ⅲ기분별비교유현저차이（ P＜0.05）；자궁내막암유、무림파결전이조간RI화VI치차이현저（ P＜0.05）。결론삼유능량다보륵초성기술대자궁내막량、악성병변적진단여감별진단유중요림상개치，위림상술전평고자궁내막암적분기제공료일충상대준학적신수단。
Objective To compare with blood parameters value of benign and malignant endometrial disease by three dimensional power Doppler ultrasound and find the correlation between vascularization parameters and pathological stage assessed by three dimensional power Doppler in endometrial cancer .Methods 55 women with postmenopausal bleeding were selected , resistance index (RI),pulsatility index ( PI) , peak systolic velocity ( PSV) and vascularization index ( VI) were recorded and compared .Results The PSV, PI, RI in be-nign endometrial disease were significantly higher than those in endometrial carcinoma ,and the VI in benign endometrial disease was signifi-cantly lower than that in endometrial carcinoma (P<0.05).The VI values in stage Ⅱwas significantly higher than that in stage Ⅰ,and the VI values in stage Ⅲwas significantly higher than that in stage Ⅱ.There were significant differences between with and without lymph node metastasis in RI and VI(P<0.05).Conclusions Three dimensional power Doppler has important value in diagnosis and differential diagno -sis on the endometrial disease and provides a new method for evaluating clinical stage of endometrial cancer before operation .
