目的：探讨麻痹性痴呆患者的临床表现、实验室检查、影像学特征，为早期诊断提供依据。方法对21例麻痹性痴呆患者的临床资料进行回顾性分析。结果本组患者均无自知力，并伴有精神病性症状、记忆力下降，部分患者伴有共济失调等症状；简易智力状态检查量表评分为（10±3）分，实验室检查梅毒螺旋体血凝试验及快速血浆反应素环状卡片试验均为阳性；其中13例患者行头颅C T检查，提示脑萎缩3例。结论麻痹性痴呆早期临床表现复杂多样，容易导致误诊误治，应详细询问病史，重视梅毒血清和脑脊液抗体检查，并结合影像学提高确诊率。
목적：탐토마비성치태환자적림상표현、실험실검사、영상학특정，위조기진단제공의거。방법대21례마비성치태환자적림상자료진행회고성분석。결과본조환자균무자지력，병반유정신병성증상、기억력하강，부분환자반유공제실조등증상；간역지력상태검사량표평분위（10±3）분，실험실검사매독라선체혈응시험급쾌속혈장반응소배상잡편시험균위양성；기중13례환자행두로C T검사，제시뇌위축3례。결론마비성치태조기림상표현복잡다양，용역도치오진오치，응상세순문병사，중시매독혈청화뇌척액항체검사，병결합영상학제고학진솔。
Objective To explore the clinical manifestations ,laboratory examina-tion and imaging characteristics of dementia paralytica (DP) patients in order to provide basis for early diagnosis .Methods Retrospective analyses of clinical data were carried out in 21 DP patients .Results All patients had no insight ,were accompanied with psychotic symptoms and hypomnesia ,and partial patients with ataxia and so on ;the MMSE score was 10 ± 3 ,both laboratory treponemal hemagglutination and rapid plasma reagin test were positive ;three of 13 patients performed brain CT suggested brain atrophy .Conclusion Early clinical manifestations DP patients are complicated and diversified ,result in diagnostic errors and mistreatment easily ;in order to improve con-firmed diagnosis rate case history should be carefully questioned ,serum and cerebrospinal fluid syphilis an-tibody examination valued ,and combined with imaging .
