目的了解外排泵系统在鲍曼不动杆菌耐碳青霉烯类抗生素机制中所起的作用,观察泮托拉唑逆转鲍曼不动杆菌亚胺培南耐药的作用,为多重耐药鲍曼不动杆菌感染的治疗提供实验室数据。方法收集亚胺培南中介或耐药的鲍曼不动杆菌36株,采用琼脂平板二倍稀释法重新测定亚胺培南对实验菌株的最低抑菌浓度(MIC),同批测定加入外排泵抑制剂羰基氰氯苯腙(CCCP)、泮托拉唑(PTZ)后抗菌药物的MIC值。结果36株实验株中加入羰基氰氯苯腙(CCCP)后亚胺培南MIC值明显降低的有13株(MIC降低4倍或以上),在这13株鲍曼不动杆菌(外排泵阳性组)中加入泮托拉唑后亚胺培南MIC值也有明显下降(P<0.05)。结论鲍曼不动杆菌杆对碳青霉烯类药物的主动外排在其耐药机制中起着非常重要的作用,外排泵阳性菌株中加入泮托拉唑后亚胺培南MIC值降低,增加了鲍曼不动杆菌对亚胺培南的敏感性,具有一定的应用前景。

목적료해외배빙계통재포만불동간균내탄청매희류항생소궤제중소기적작용,관찰반탁랍서역전포만불동간균아알배남내약적작용,위다중내약포만불동간균감염적치료제공실험실수거。방법수집아알배남중개혹내약적포만불동간균36주,채용경지평판이배희석법중신측정아알배남대실험균주적최저억균농도(MIC),동비측정가입외배빙억제제탄기청록분종(CCCP)、반탁랍서(PTZ)후항균약물적MIC치。결과36주실험주중가입탄기청록분종(CCCP)후아알배남MIC치명현강저적유13주(MIC강저4배혹이상),재저13주포만불동간균(외배빙양성조)중가입반탁랍서후아알배남MIC치야유명현하강(P<0.05)。결론포만불동간균간대탄청매희류약물적주동외배재기내약궤제중기착비상중요적작용,외배빙양성균주중가입반탁랍서후아알배남MIC치강저,증가료포만불동간균대아알배남적민감성,구유일정적응용전경。
Objective To understand the role and distribution of multidrug ef lux pumps in clinical isolates of carbapenems resistant Acinetobacter baumannii, investigate the reversal resistant in Acinetobacter baumanni by pantoprazole, and thus to provide information for selection of antibiotics for the treatment of mutlidrug-resistant infections. Methods A total of 36 non-duplicate clinical isolates were recovered and antimicrobial susceptibility was conducted by agar dilution methods with or without the inclusion of pantoprazole or carbonyl cyanide-m-chlorophenylhydrazone (CCCP) . Results The results showed that 13 isolates after treatment of imipenem with CCCP were positive of ef lux mechanism(their MIC reduced by 4 times or more). The same result was deteced when pantoprazole and imipenem were added to the dilution ( <0.05). Conclusion Ef lux plays a important role in carbapenems resistance in the clinical isolates of A.baumanni . Pantoprazole could enhance antibiotic acticities of imipenem to Acinetobacter baumanni including multidrug ef lux pumps,and has certain application prospect.
