目的:探讨外周血血小板p-选择素(CD62p)、D二聚体及超敏C反应蛋白在下肢深静脉血栓形成(LDVT)中的临床意义。方法选择2011年1月~2012年1月于广州中医药大学第一附属医院确诊为LDVT的32例患者为治疗组,选择于广州中医药大学第一附属医院行健康体检的30例健康人为对照组。检测对照组及治疗组治疗前后CD62p、D二聚体及超敏C反应蛋白水平,LDVT患者治疗前后两侧下肢周径差。结果治疗前LDVT组患者血清的CD62p[(25.80±4.26)豫]、D二聚体[(2495±95)μg/L]和超敏C反应蛋白[(5.79±0.75)mg/L]均显著高于对照组,治疗3 d后,三者仍显著高于对照组,差异均有统计学意义(P<0.05)。但其中D二聚体和超敏C反应蛋白均较治疗前显著降低,且在治疗7 d后较治疗3 d后进一步降低,差异均有统计学意义(P<0.05)。患者两侧下肢周径差随着治疗时间的延长而显著减小。结论 CD62p、D二聚体及超敏C反应蛋白是在LDVT及降解过程的不同特异性指标,随着LDVT的发展而变化,可以作为LDVT的早期诊断依据。

목적:탐토외주혈혈소판p-선택소(CD62p)、D이취체급초민C반응단백재하지심정맥혈전형성(LDVT)중적림상의의。방법선택2011년1월~2012년1월우엄주중의약대학제일부속의원학진위LDVT적32례환자위치료조,선택우엄주중의약대학제일부속의원행건강체검적30례건강인위대조조。검측대조조급치료조치료전후CD62p、D이취체급초민C반응단백수평,LDVT환자치료전후량측하지주경차。결과치료전LDVT조환자혈청적CD62p[(25.80±4.26)예]、D이취체[(2495±95)μg/L]화초민C반응단백[(5.79±0.75)mg/L]균현저고우대조조,치료3 d후,삼자잉현저고우대조조,차이균유통계학의의(P<0.05)。단기중D이취체화초민C반응단백균교치료전현저강저,차재치료7 d후교치료3 d후진일보강저,차이균유통계학의의(P<0.05)。환자량측하지주경차수착치료시간적연장이현저감소。결론 CD62p、D이취체급초민C반응단백시재LDVT급강해과정적불동특이성지표,수착LDVT적발전이변화,가이작위LDVT적조기진단의거。
Objective To explore the clinical value of cluster of differentiation 62 platelet (CD62p), D-dipolymer (D-D) and hypersensitive C-reactive protein (hCRP) from perihpheral blood of lower deep venous thrombosis. Methods From January 2011 to January 2012, in the First Affiliated Hospital of Guangzhou Chinese Medical University, 32 pa-tients with LDVT (LDVT group) and 30 healthy people (control group) were selected. The concentration of CD62p, D-D and hCRP in both groups and the difference of perimeters between the lower limbs before and 3, 7 days after the treat-ment in treatment group were recorded. Results The concentration of CD62p [(25.80±4.26)%], D-D [(2495±95)μg/L] and hCRP [(5.79±0.75)mg/L] of LDVT before the treatment were statistically higher than those of control group, the differences were statistically significant (P < 0.05). The concentration of D-D and hCRP decreased vitally after the treatment 3, 7 days, but they were still higher than those of control group, the differences were statistically significant (P < 0.05). The the difference of perimeters between the lower limbswere statistically reduced after the treatment. Conlusion CD62p, D-D and hCRP are the specific indicators of the thrombosis and thromclasis in lower deep vein and can be taken as the early diagnostic evidences of LDVT.
