目的：分析良性阵发性位置性眩晕（BPPV）的误诊误治及漏诊情况，寻找其原因及对策。方法：收集某医院由耳鼻喉科听力检查室经Dix-Hallpike或Roll test诱发试验确诊的BPPV患者资料，调查、分析其诊治过程。结果：经该室确诊的BPPV患者共128例，其中32例为首次就诊并确诊，46例有误治误治经历，50例首次就诊后未获任何诊断意向。结论：BPPV是眩晕患者最常见的原因，然而首诊确诊率却很低，误诊误治及漏诊情况也很常见。临床医生应学会Dix-Hallpike及Roll test诱发试验的检查操作，尽量减少BPPV的误诊漏诊以及患者的身心痛苦和经济负担。
목적：분석량성진발성위치성현훈（BPPV）적오진오치급루진정황，심조기원인급대책。방법：수집모의원유이비후과은력검사실경Dix-Hallpike혹Roll test유발시험학진적BPPV환자자료，조사、분석기진치과정。결과：경해실학진적BPPV환자공128례，기중32례위수차취진병학진，46례유오치오치경력，50례수차취진후미획임하진단의향。결론：BPPV시현훈환자최상견적원인，연이수진학진솔각흔저，오진오치급루진정황야흔상견。림상의생응학회Dix-Hallpike급Roll test유발시험적검사조작，진량감소BPPV적오진루진이급환자적신심통고화경제부담。
Objective:The aim of this study is to analyze the misdiagnosis and missed diagnosis on benign paroxysmal positional vertigo (BPPV),and then find out its reasons and countermeasures. Methods:Medical records from patients who have been diagnosed as patients with BPPV by dix-hallpike or roll-test inducement experiment at hearing checking room of department of otorhinolaryngology were collected. And their diagnosing and treating processes were investigated and analyzed. Result:There are 128 patients with BPPV diagnosed by the hearing checking room, among which 32 patients went to clinic for the first time and were diagnosed as patients with BPPV. 46 patients have been misdiagnosed and mistreated and 50 patients haven`t got diagnosing intention after going to clinic for the first time. Conclusion:BPPV is the commonest reason for vertigo patients, however the defining rate of the first diagnosis is very low, and misdiagnosis and mistreat or missed diagnosis is rather common. So clinical doctors should deepen the understanding of the disease and try to reduce misdiagnosis and missed diagnosis on BPPV.
