目的：针对日益增长的高龄、病残、失智、失能老人护理现状，改变传统的家庭养老模式，创新“医养融合”老年护理机构和工作内容。方法：以责任制整体护理理论基础为指导，为入住老人提供优质的医疗护理、生活护理服务，即“医养融合”老年护理服务。结果：“医养融合”老年护理完整地覆盖了生命周期。医疗、养老相向而行是一种1+1>2的护理模式。结论：集“托老、诊疗、护理、康复、临终关怀”为一体的具有专业化特色的老年护理机构，即“医养融合”老年护理中心，创新了医疗机构和养老机构合作新模式，推动了“医养融合”老年护理的发展。

목적：침대일익증장적고령、병잔、실지、실능노인호리현상，개변전통적가정양로모식，창신“의양융합”노년호리궤구화공작내용。방법：이책임제정체호리이론기출위지도，위입주노인제공우질적의료호리、생활호리복무，즉“의양융합”노년호리복무。결과：“의양융합”노년호리완정지복개료생명주기。의료、양로상향이행시일충1+1>2적호리모식。결론：집“탁로、진료、호리、강복、림종관부”위일체적구유전업화특색적노년호리궤구，즉“의양융합”노년호리중심，창신료의료궤구화양로궤구합작신모식，추동료“의양융합”노년호리적발전。
Objective: To change the traditional family elderly care mode and to innovate "medical and old-age care fusion" geriatric nursing institute and working content according to the current nursing status of the increasing age, disability, dementia, frail elderly. Methods: Guided by primary holistic nursing theory, we provided the elderly with high quality medical care and life care which was called "medical and old-age care fusion" geriatric nursing service. Results: the "medical and old-age care fusion" geriatric nursing completely covered the life cycle and it was a nursing mode of 1+1>2. Conclusion: The "medical and old-age care fusion" geriatric nursing institute combined the special characteristics of "elderly care, treatment, nursing, rehabilitation, palliative care" in an aged care center and promote the development of elderly care. This is the only way of developing elderly care in the aging society.
