目的:观察芪苈强心胶囊联合卡维地洛对慢性心力衰竭患者核因子κB(NF κB)及诱导型一氧化氮合酶(iNOS)水平的影响。方法120例慢性心力衰竭患者随机分为常规组、卡维地洛组、芪苈强心胶囊联合卡维地洛组(联合组),各40例。采用荧光定量PCR法和酶联免疫分析方法分别测定各组治疗前及治疗12周后血液 NF κB和 iNOS的水平,测定左室射血分数,并进行6 min 步行试验。结果治疗12周后,卡维地洛组、芪苈强心胶囊联合卡维地洛组血液NF κB和iNOS水平均较常规组明显下降(P<0.01),6 min步行距离显著增加(P<0.05),左室射血分数明显提高(P<0.05);卡维地洛组与联合组相比较,血液 NF κB和iNOS水平下降(P<0.05),而6 min步行距离、左室射血分数无统计学意义。结论芪苈强心胶囊联合卡维地洛治疗慢性心力衰竭可提高患者运动耐力,并改善心脏功能,推测芪苈强心胶囊联合卡维地洛可能通过抑患者炎性因子的表达而改善心功能。

목적:관찰기력강심효낭연합잡유지락대만성심력쇠갈환자핵인자κB(NF κB)급유도형일양화담합매(iNOS)수평적영향。방법120례만성심력쇠갈환자수궤분위상규조、잡유지락조、기력강심효낭연합잡유지락조(연합조),각40례。채용형광정량PCR법화매련면역분석방법분별측정각조치료전급치료12주후혈액 NF κB화 iNOS적수평,측정좌실사혈분수,병진행6 min 보행시험。결과치료12주후,잡유지락조、기력강심효낭연합잡유지락조혈액NF κB화iNOS수평균교상규조명현하강(P<0.01),6 min보행거리현저증가(P<0.05),좌실사혈분수명현제고(P<0.05);잡유지락조여연합조상비교,혈액 NF κB화iNOS수평하강(P<0.05),이6 min보행거리、좌실사혈분수무통계학의의。결론기력강심효낭연합잡유지락치료만성심력쇠갈가제고환자운동내력,병개선심장공능,추측기력강심효낭연합잡유지락가능통과억환자염성인자적표체이개선심공능。
Objective To observe the influence of carvedilol combined with Qili Qiangxin capsules(QQC)on the levels of blood nuclear factor kappa B (NF κB),inducible nitric oxide synthase (iNOS)in patients with early congestive heart failure(CHF). Methods One hundred and twenty patients with early CHF were randomly divided into routine treatment group,carvedilol group,and carvedilol combined with QQC treatment groups,and 40 cases were included in each group.After medication for 12 weeks,the blood levels of iNOS and NF κB were detected with enzyme linked immunosorbent assay and fluorescent quantitation PCR,respectively. The left ventricular ej ection fraction (LVEF)and walking distance in 6 minutes were detected at the same time.Results After 1 2 weeks drug therapy,the blood levels of NF κB and iNOS were decreased significantly (P<0.01),and walking distance in 6 minutes and LVEF were increased(P<0.05)in carvedilol group and carvedilol combined with QQC groups compared with that in routine treat-ment group.The blood levels of NF κB and iNOS were decreased (P<0.05)in carvedilol group compared with carvedilol combined with QQC group,while there were no significantly differences of walking distance in 6 minutes and LVEF between the two groups. Conclusion QQC combined with carvedilol could improve the exercise tolerance and improve cardiac function of early CHF patients and it might due to its inhibiting effects on the expression of inflammation factors.
