目的:探讨多排螺旋 CT 对早期周围型小肺癌的诊断价值。方法收集近4年我院经穿刺活检或手术切除病理证实,同时具有完整多排螺旋 CT 检查资料的43例周围型小肺癌患者,对该疾病影像学特点(分叶征、毛刺征、支气管充气征、血管集束征、空泡征、胸膜凹陷征)进行系统的分析。结果本组43例病变均为直径≤20 mm 的孤立性结节,均行胸部多排螺旋 CT 扫描、横断面高分辨 CT(HRCT)扫描、动态 CT 增强扫描及胸部容积多平面重建(MPR),所有患者病变至少具有该疾病影像学特点中的2~3个征象,可作为该病影像及临床诊断的可靠依据。结论多排螺旋 CT 对周围型小肺癌能够准确定位,具有较高的检出率和应用价值,能够为临床诊断及治疗提供客观的参考及依据。

목적:탐토다배라선 CT 대조기주위형소폐암적진단개치。방법수집근4년아원경천자활검혹수술절제병리증실,동시구유완정다배라선 CT 검사자료적43례주위형소폐암환자,대해질병영상학특점(분협정、모자정、지기관충기정、혈관집속정、공포정、흉막요함정)진행계통적분석。결과본조43례병변균위직경≤20 mm 적고립성결절,균행흉부다배라선 CT 소묘、횡단면고분변 CT(HRCT)소묘、동태 CT 증강소묘급흉부용적다평면중건(MPR),소유환자병변지소구유해질병영상학특점중적2~3개정상,가작위해병영상급림상진단적가고의거。결론다배라선 CT 대주위형소폐암능구준학정위,구유교고적검출솔화응용개치,능구위림상진단급치료제공객관적삼고급의거。
Objective To explore the diagnostic value of multi-slice spiral CT in small peripheral lung cancer( SPLC). Methods The complete data of multi-slice spiral CT of 43 cases with SPLC confirmed by biopsy or postoperative pathological study were collected and a systematic analysis was made to the imaging features of the disease. Results The lesions in the 43 cases were all the isolated nodules with the diameter of ≤20 mm;all the cases underwent chest multi-slice spiral CT scanning,HRCT scanning, dynamic enforced CT scanning and MPR of chest volume. The scanning showed out at least 2 to 3 imaging features of the disease,which provided a reliable basis for imaging and clinical diagnosis. Conclusions Multi-slice spiral CT is accurate in the location of SPLC and is of high detection rate and application value as it can provide objective reference and basis for clinical diagnosis and treatment.
