目的：观察手术切除加术后肝动脉和肝静脉双插管化疗治疗原发性肝癌合并门静脉癌栓的治疗效果。方法收集我院2000年1月至2011年1月期间15例肝癌合并门静脉癌栓患者临床资料,15例均采用肝癌切除加门静脉癌栓取栓术,术后采用肝动脉和肝静脉双插管微量泵灌注化疗。结果15例患者均成功完成手术,术后1、2和3年的生存率分别为73.3%、40.0%、20.0%,平均生存时间24.8个月。结论对于肝癌合并门静脉癌栓患者,手术切除后使用肝动脉和门静脉双插管微量泵灌注化疗可有效提高治疗效果,延长患者生存时间。
목적：관찰수술절제가술후간동맥화간정맥쌍삽관화료치료원발성간암합병문정맥암전적치료효과。방법수집아원2000년1월지2011년1월기간15례간암합병문정맥암전환자림상자료,15례균채용간암절제가문정맥암전취전술,술후채용간동맥화간정맥쌍삽관미량빙관주화료。결과15례환자균성공완성수술,술후1、2화3년적생존솔분별위73.3%、40.0%、20.0%,평균생존시간24.8개월。결론대우간암합병문정맥암전환자,수술절제후사용간동맥화문정맥쌍삽관미량빙관주화료가유효제고치료효과,연장환자생존시간。
Objective To observe the treatment effectiveness of Operation treatment followed hepatic artery and portal vein chemotherapy for hepatocellular carcinoma (HCC) with portal vein tumor thrombi (PVTT) . Methods We collected the clinic data of 15 cases suffered from HCC with tumor thrombi in portal vein in our hospital from January 2000 to January 2011,there are 15 patients underwent hepatectomy and portal vein tumor thrombi extraction and treat with chemotherapy via hepatic artery and por-tal vein. Results In operation group,the 1,2and 3 year survival rates were 73. 3% ,40. 0% and 20. 0% respectively in the 15 patients,and average survival time is 24. 8 months. Conclusion Surgical intervention is effective in the treatment of HCC with PVTT, hepatic artery and portal vein chemotherapy after operation can enhance the effect and prolong the live time.
