目的：探索以癫痫和精神异常为首发的糖尿病临床特点、误诊原因及诊断体会。方法我院2002-02-2012-02收治的各种类型癫痫76例临床资料，其中17例以癫痫样发作和精神异常为首发，完善相关医技检查后，确诊为糖尿病性癫痫、糖尿病性精神病。结果首诊误诊率为22.4％，分别被误诊为癫痫15例，精神病2例。结论临床医生应提高对以癫痫样、精神病样发作为首发的糖尿病认识，当遇到类似症状患者除想到癫痫精神病外，还应想到糖尿病的可能，及时选做尿糖、血糖监测，并给予胰岛素等降糖药治疗，可明确诊断。
목적：탐색이전간화정신이상위수발적당뇨병림상특점、오진원인급진단체회。방법아원2002-02-2012-02수치적각충류형전간76례림상자료，기중17례이전간양발작화정신이상위수발，완선상관의기검사후，학진위당뇨병성전간、당뇨병성정신병。결과수진오진솔위22.4％，분별피오진위전간15례，정신병2례。결론림상의생응제고대이전간양、정신병양발작위수발적당뇨병인식，당우도유사증상환자제상도전간정신병외，환응상도당뇨병적가능，급시선주뇨당、혈당감측，병급여이도소등강당약치료，가명학진단。
Objective To explore the clinical features ,misdiagnosis factor and diagnosis realization on diabetics with initial symptoms of epilepsy and mental abnormality .Methods By reviewing clinical data from 76 cases of various types of epilepsy who received and cured in our hospital during February of 2002 and February of 2012 ,17 cases with epileptic seizures and men-tal abnormality as the initial symptoms were diagnosed with diabetic epilepsy and diabetic psychosis after comprehensive medical check .Results The misdiagnosis rate was 22 .4% at first visit and diagnosis ,15 cases were misdiagnosed with epilepsy and 2 cases were psychosis .Conclusion Clinicians should improve understanding of diabetes with epileptic seizures and sample psy-chosis attack as the initial symptoms ,so that they could take differential diagnosis of diabetes into account besides epilepsy and psychosis ,by making urine glucose examination and blood-sugar monitor timely ,as well as antidiabetic therapy such as insulin injection .
