目的：探讨两种不同手术方式对于慢性硬膜下血肿患者治疗效果的临床差异性。方法将收治的623例慢性硬膜下血肿手术患者的临床资料作回顾性分析及对比研究，其中335例采用YL-1微创穿刺手术，288例采用钻孔引流手术。结果在手术时间、住院时间方面，两种术式有统计学差异（ P＜0．05）。两种手术方式的治愈率、并发症发生率无明显差异（P＞0．05），穿刺组有1例患者因新鲜出血行骨瓣开颅。结论对于慢性硬膜下血肿患者，钻孔引流术是经典术式，微创穿刺术亦有明确疗效，且操作方法简便、创伤小，住院周期及花费少，在临床应用中可进一步推广。

Objective To explore the curative effect of two operation methods in the treatment of patients with chronic subdural hematoma ( CSDH ) .Methods 623 patients with chronic subdural hematoma were underwent surgery,including 335 patients by hard tunnel minimally invasive technique (YL-1 type) and 288 patients by burr hole irrigation with drainage .Results As compared with trepanation and drainage operation ,YL-1 operation presented a shorter operation time and hospitalization time (P <0.05).There were no significant differences between the two operation methods regarding postoperative complications and effective rate (P>0.05).In YL-1 group, 1 case of postoperative hemorrhage was underwent traditional fronto-temporal craniotomy .Conclusions Trepanation and drainage operation has been regarded as classic operation method for CSDH ,the curative effect of YL-1 type needle in patients with CSDH is also exact , which can effectively reduce the hospitalization time , alleviate the patients'suffering, so it is worthy of clinical application .
