目的：探讨大骨瓣开颅减压术治疗对冲性重型颅脑损伤的疗效。方法：选择2005年2月～2013年10月我院收治的120例对冲性重型颅脑损伤患者，并依所接受的不同治疗方案随机分组，对照组患者60例、采用常规骨瓣开颅手术治疗，观察组患者60例、采用大骨瓣开颅减压手术治疗，比较疗效。结果：经临床观察，观察者的病死率、并发症发生情况明显少于对照组，组间差异（P＜0.05），有统计学意义；且观察组患者的术后颅内压水平明显低于对照组，组间差异（P＜0.05），有统计学意义。结论：大骨瓣开颅减压术是临床治疗对冲性重型颅脑损伤的有效方式，疗效显著，安全性高。
목적：탐토대골판개로감압술치료대충성중형로뇌손상적료효。방법：선택2005년2월～2013년10월아원수치적120례대충성중형로뇌손상환자，병의소접수적불동치료방안수궤분조，대조조환자60례、채용상규골판개로수술치료，관찰조환자60례、채용대골판개로감압수술치료，비교료효。결과：경림상관찰，관찰자적병사솔、병발증발생정황명현소우대조조，조간차이（P＜0.05），유통계학의의；차관찰조환자적술후로내압수평명현저우대조조，조간차이（P＜0.05），유통계학의의。결론：대골판개로감압술시림상치료대충성중형로뇌손상적유효방식，료효현저，안전성고。
[ABSTRACT]Objective: To investigate the therapeutic effect of large trauma craniotomy in the treatment of contrecoup severe craniocerebral injury.Methods:120 patients with contrecoup severe craniocerebral injury patients from 2005 February ~2013 year in October in our hospital, and randomly grouped according to different treatment received, the control group of 60 cases, treated with routine craniectomy operation, patients in the observation group, 60 cases with large bone flap craniotomy decompression operation treatment,curative effect.Results:The clinical observation, the mortality rate of the observer, and incidence of complications was significantly less than that of the control group, the differences between groups (P<0.05), there is statistical significance; intracranial pressure and the observation group was significantly lower than the control group, the differences between groups (P<0.05), there was statistical significance.Conclusion:Large trauma craniotomy is an effective way, the clinical treatment of contrecoup craniocerebral injury has obvious curative effect, high safety.
