目的:探讨腹腔镜下根治手术配合空肠间置代胃术治疗胃癌的临床疗效。方法:将126例2012年6月至2013年6月期间我院收治的胃癌患者按数字随机法分为A组和B组,每组患者63例。A组患者进行腹腔镜下根治手术配合空肠间置代胃术治疗。B组患者行全胃切除食管空肠袢式吻合术,比较两组手术时间、并发症、术后营养学指标和生活质量等。结果:治疗后A组体重、总蛋白、血红蛋白和白蛋白等营养学指标水平分别为(57.96±24.16)kg、(71.58±8.14)g/dl、(15.21±2.65)g/dl和(39.47±4.05)g/dl,均高于B组的(46.62±14.78)kg、(57.42±7.95)g/dl、(8.43±2.47)g/dl和(30.01±2.86)g/dl,且A组躯体功能、躯体角色、肢体疼痛等8个维度的生活质量评分得分亦较B组明显提高,差异有统计学意义(P<0.05)。A组手术时间为(40.08±5.54)min,少于B组的(49.84±6.51)min,差异有统计学意义(t=4.25,P<0.05)。A组术后总并发症发生率较B组降低(P<0.05)。结论:腹腔镜下根治手术配合空肠间置代胃术治疗胃癌的手术时间短,患者并发症发生少,患者全营养状况和生活质量均改善。

목적:탐토복강경하근치수술배합공장간치대위술치료위암적림상료효。방법:장126례2012년6월지2013년6월기간아원수치적위암환자안수자수궤법분위A조화B조,매조환자63례。A조환자진행복강경하근치수술배합공장간치대위술치료。B조환자행전위절제식관공장번식문합술,비교량조수술시간、병발증、술후영양학지표화생활질량등。결과:치료후A조체중、총단백、혈홍단백화백단백등영양학지표수평분별위(57.96±24.16)kg、(71.58±8.14)g/dl、(15.21±2.65)g/dl화(39.47±4.05)g/dl,균고우B조적(46.62±14.78)kg、(57.42±7.95)g/dl、(8.43±2.47)g/dl화(30.01±2.86)g/dl,차A조구체공능、구체각색、지체동통등8개유도적생활질량평분득분역교B조명현제고,차이유통계학의의(P<0.05)。A조수술시간위(40.08±5.54)min,소우B조적(49.84±6.51)min,차이유통계학의의(t=4.25,P<0.05)。A조술후총병발증발생솔교B조강저(P<0.05)。결론:복강경하근치수술배합공장간치대위술치료위암적수술시간단,환자병발증발생소,환자전영양상황화생활질량균개선。
[ABSTRACT]Objective: To investigate the clinical effect of laparoscopic radical surgery and jejunum interposition treating gastric cancer.Methods:126 gastric cancer patients in our hospital from June, 2012 to June, 2013 were divided into group A and group B according to digital random method, 63 cases for each group. Group A was treated with laparoscopic radical surgery and jejunum interposition and Group B was treated with braun esophagojejunostomy. Operation time, complications, and postoperative nutrition index and life quality after treatment were compared.Results:Weight, total protein, albumin and hemoglobin level of group A after treatment are (57.96±24.16) kg, g/dl (71.58±8.14), (15.21±2.65) g/dl and (39.47±4.05) g/dl respectively, which are higher than the (46.62±14.78) kg, (57.42±7.95) g/dl, g/dl (8.43±2.47) and (30.01± 2.86) g/dl of group B,and score points of physical function, physical character, body pain and other dimensions of life quality of group A are higher than that of group B, and the difference is statistically signiifcant (P<0.05). Operating time of group A is (40.08±5.54) min, which is shorter than the (49.84±6.51) min of group B (t=4.25,P<0.05).Total postoperative complication rate of group A is lower than that of group B (P<0.05).Conclusion:Laparoscopic radical surgery and jejunum interposition treating gastric cancer is worthy of clinical use for it can shorter operation time, reduce complications, and improve nutritional status and life quality.
