目的:观察无创通气联合家庭氧疗对慢性肺源性心脏病心肺功能失代偿期患者的影响。方法:此为回顾性研究,对于门诊107例慢性肺源性心脏病失代偿期患者进行为期一年的观察,因经济原因产生三组病人,一组为不进行长期家庭氧疗的患者(A组),一组为只进行家庭氧疗的患者(B组),另一组为长期进行家庭氧疗及间断无创机械通气的患者(C组)。然后每半年比较治疗前后动脉血气(PaO2、PaCO2)、肺动脉收缩压(SPAP)、肺功能指标(FEV1和FVC)的改善程度、急性加重的住院频率。结果:间断无创机械通气联合家庭氧疗组较另两组在动脉血PaO2、PaCO2、SPAP、FVC及住院频率方面有明显改善(P<0.05),而FEV1与对照组相比无明显差异(P均>0.05)。结论:无创通气能降低肺心病心肺功能失代偿期患者的肺动脉压,显著改善肺功能,降低急性加重住院率。

목적:관찰무창통기연합가정양료대만성폐원성심장병심폐공능실대상기환자적영향。방법:차위회고성연구,대우문진107례만성폐원성심장병실대상기환자진행위기일년적관찰,인경제원인산생삼조병인,일조위불진행장기가정양료적환자(A조),일조위지진행가정양료적환자(B조),령일조위장기진행가정양료급간단무창궤계통기적환자(C조)。연후매반년비교치료전후동맥혈기(PaO2、PaCO2)、폐동맥수축압(SPAP)、폐공능지표(FEV1화FVC)적개선정도、급성가중적주원빈솔。결과:간단무창궤계통기연합가정양료조교령량조재동맥혈PaO2、PaCO2、SPAP、FVC급주원빈솔방면유명현개선(P<0.05),이FEV1여대조조상비무명현차이(P균>0.05)。결론:무창통기능강저폐심병심폐공능실대상기환자적폐동맥압,현저개선폐공능,강저급성가중주원솔。
Aim: to investigate the effect of Noninvasive ventilation combined with home oxygen therapy on heart and lung functions in the chronic pulmonary heart disease (CPHD) patients at the subjective perception and objective indicators. Methods: A total of 107 patients with decompensated CPHD were enroled in a one-year plan and divided into three groups for economic factors: control group (A), home oxygen therapy group (B) and home oxygen therapy combined with discontinuity Noninvasive mechanical ventilation group (C). Arterial blood gas (PaO2、PaCO2)、pulmonary arterial systolic pressure(SPAP)、pulmonary function (FEV1、FVC)and acute exacerbations (Frequency of hospitalization,FH) were monitored every half an year and compared before and after treatment. Results: home oxygen therapy combined with discontinuity Noninvasive mechanical ventilation significantly improved the therapeutic effects as indicated by PaO2、PaCO2、SPAP、FVC and FH. However, FEV1 values showed no significant differences between trail and control group.Conclusion: Noninvasive ventilation decreased SPAP and acute exacerbations、improved pulmonary functions in patients with decompensated CPHD.
