目的:研究乳腺癌钼靶X线征象与乳腺癌病理学预后因子的相关性。方法:对420例乳腺癌患者术前行钼靶X线分析,术后病理测定肿瘤的组织学分级、肿瘤的脉管浸润(LVI)、雌激素受体(ER)、孕激素受体(PR)、人类表皮生长因子(Her-2)、肿瘤增殖指标(Ki-67)、抑癌基因P53的表达情况,对钼靶X线征象与临床病理学预后因子的相关性采用单因素和多因素Logistic回归分析。结果:钼靶X线征象中肿块伴随钙化,肿块边缘模糊与高组织学分级相关;肿块伴随钙化,肿块边缘清晰与Ki-67阳性相关;单独钙化,肿块伴随钙化,大肿块(>5cm)与Her-2阳性相关;无影像征象与ER阳性、P53和LVI相关。结论:乳腺癌部分钼靶X线征象与临床病理学预后因子有相关性,有一定的预测价值,为乳腺癌的辅助治疗提供了一定的信息。

목적:연구유선암목파X선정상여유선암병이학예후인자적상관성。방법:대420례유선암환자술전행목파X선분석,술후병리측정종류적조직학분급、종류적맥관침윤(LVI)、자격소수체(ER)、잉격소수체(PR)、인류표피생장인자(Her-2)、종류증식지표(Ki-67)、억암기인P53적표체정황,대목파X선정상여림상병이학예후인자적상관성채용단인소화다인소Logistic회귀분석。결과:목파X선정상중종괴반수개화,종괴변연모호여고조직학분급상관;종괴반수개화,종괴변연청석여Ki-67양성상관;단독개화,종괴반수개화,대종괴(>5cm)여Her-2양성상관;무영상정상여ER양성、P53화LVI상관。결론:유선암부분목파X선정상여림상병이학예후인자유상관성,유일정적예측개치,위유선암적보조치료제공료일정적신식。
Objective The purpose of this study was to correlate mammographic findings with prognostic factors in patients with breast cancer. Methods findings in 420 patients with breast cancers were retrospectively evaluated. Pathology reports relating to histological grade, lymphovascular invasion (LVI), oestrogen receptor (ER) status, progesterone receptor (PR) status and Her-2/neu status ,P53 status,Ki-67 status were reviewed .Univariate and multivariate logistic regression was applied to analyze the association between mammographic findings and prognostic factors in patients with breast cancer.Results On mammography, Associated calcification were associated with many poor prognostic factors:high histological grade, positive Her-2 and Ki-67,Indistinct margins was associated with high histological grade,distinct margins were associated with positive Ki-67, calcifications alone、The sizes of masses(>5cm) were associated with positive Her-2, None of finds was associated with positiveER、 P53 and LVI.Conclusion Several mammographic features can have a prognostic value in the subsequent treatment of patients with breast cancer.
