目的：探讨鼻内镜下泪囊鼻腔造孔术失败的发生原因及预防措施。方法回顾分析9例鼻内镜下泪囊鼻腔造孔术的失败原因。结果失败病例包括小泪囊3例、伴鼻部疾病如鼻中隔偏曲或鼻息肉3例、外伤性鼻泪管阻塞1例、未按医嘱使用类固醇药物喷鼻及按时随访2例。结论鼻内窥镜下行鼻腔泪囊造孔术是治疗慢性泪囊炎的较好方法，熟练掌握鼻泪系统的解剖学知识、重视围手术期处理及手术技巧、妥善处理小泪囊、同期治疗鼻部疾病、减少造孔口处肉芽增生等可减少手术失败的发生。

목적：탐토비내경하루낭비강조공술실패적발생원인급예방조시。방법회고분석9례비내경하루낭비강조공술적실패원인。결과실패병례포괄소루낭3례、반비부질병여비중격편곡혹비식육3례、외상성비루관조새1례、미안의촉사용류고순약물분비급안시수방2례。결론비내규경하행비강루낭조공술시치료만성루낭염적교호방법，숙련장악비루계통적해부학지식、중시위수술기처리급수술기교、타선처리소루낭、동기치료비부질병、감소조공구처육아증생등가감소수술실패적발생。
Objective :Discuss the causes and prevention measures on the failure of nasal endoscopic dacryocystorhinostomy.Methods:Etrospective analysis 9 failure casues of nasal endoscopic dacryocystorhinostomy.Results:Failure cases include 3 cases of smal lacrimal sac ,3 cases with nasal diseases such as nasal septum deviation or nasal polyps , 1 case of traumatic nasolacrimal duct obstruction,2 cases who not according to the doctor's advice to use steroids nasal spray and folowe-up on time.Conclusion :Nasal endoscopic dacryocystorhinostomy is the better method for treating chronic dacryocystitis,proficient master the anatomy of the nasal system,pay attention to perioperative management and surgical techniques,properly handle the smal lacrimal sac,treat the nasal diseases at the same time, reduce granulation hyperplasia on the created orifice ,and so on ,can reduce the failure.
