目的:探讨经头皮冠状切口或睑结膜切口入路,应用钛网支架重建眶底缺损的临床疗效。方法根据CT扫描和三维成像诊断,对18例眶底缺损患者进行微型钛网内固定术重建眶底缺损,其中10例为眶底伴邻近颅面骨折,采用头皮发际内冠状切口;8例为陈旧性眶底骨折,采用睑结膜切口。结果18例患者的眼球内陷和部分合并颧骨骨折的状况得到纠正,切口均愈合良好,切口位置较隐秘或未遗留瘢痕,颜面畸形得到明显改善。术后随访3~24个月,钛网支架无1例松动或移位或者其他并发症,疗效满意。结论头皮冠状切口入路以及睑结膜入路都能为术者提供足够视野与操作空间,应用微型钛网内固定术可重建眶底缺损,防止复视,术后美观效果佳。
目的:探討經頭皮冠狀切口或瞼結膜切口入路,應用鈦網支架重建眶底缺損的臨床療效。方法根據CT掃描和三維成像診斷,對18例眶底缺損患者進行微型鈦網內固定術重建眶底缺損,其中10例為眶底伴鄰近顱麵骨摺,採用頭皮髮際內冠狀切口;8例為陳舊性眶底骨摺,採用瞼結膜切口。結果18例患者的眼毬內陷和部分閤併顴骨骨摺的狀況得到糾正,切口均愈閤良好,切口位置較隱祕或未遺留瘢痕,顏麵畸形得到明顯改善。術後隨訪3~24箇月,鈦網支架無1例鬆動或移位或者其他併髮癥,療效滿意。結論頭皮冠狀切口入路以及瞼結膜入路都能為術者提供足夠視野與操作空間,應用微型鈦網內固定術可重建眶底缺損,防止複視,術後美觀效果佳。
목적:탐토경두피관상절구혹검결막절구입로,응용태망지가중건광저결손적림상료효。방법근거CT소묘화삼유성상진단,대18례광저결손환자진행미형태망내고정술중건광저결손,기중10례위광저반린근로면골절,채용두피발제내관상절구;8례위진구성광저골절,채용검결막절구。결과18례환자적안구내함화부분합병권골골절적상황득도규정,절구균유합량호,절구위치교은비혹미유류반흔,안면기형득도명현개선。술후수방3~24개월,태망지가무1례송동혹이위혹자기타병발증,료효만의。결론두피관상절구입로이급검결막입로도능위술자제공족구시야여조작공간,응용미형태망내고정술가중건광저결손,방지복시,술후미관효과가。
Objective To explore the clinical efficacy of reconstruction of orbital floor defect through coronal scalp approach or conjunctivas approach by using titanium mesh . Methods According to CT scans and three-dimensional imaging diagnosis , reconstructions of orbital floor defect were performed on 18 patients using titanium mesh . 10 cases of orbital floor defection complicated with adjacent craniofacial fractures , the patients were operated through coronary incision in the hairline scalp, while 8 cases of old orbital floor fractures were treated through eyelid conjunctiva incision . Results Enophthalmos were corrected in all 18 cases , so did some patients accompanied with zygomatic fractures. All incisions were covert and healed well without obvious scar, and facial deformity has been obviously improved. All patients were followed up for 3 to 24 months. An acceptable clinical efficacy was obtained, and titanium mesh loosening or other complications were not observed. Conclusions Coronal scalp or conjunctivas incision can provide adequate operating space for surgeons . Orbital floor fracture could be reconstructed by using titanium mesh , which resulted in a better aesthetic effect.
