目的：总结分析150例疱疹性咽峡炎的临床症状体征以及治疗转归。方法：收集2011年3月至2013年4月我院住院疱疹性咽峡炎患儿150例的临床资料并进行回顾性总结及分析。结果：150例中有29例发展成为手足口病,1例为重症；全部进行心肌酶检查,异常率85.33%(128/150),其中CKMB异常者67例占52.34%；经过营养心肌治疗7~10天后复查心肌酶24例正常,继续治疗至20天后再次复查32例恢复正常。全部进行肝功能检测,26例转氨酶升高,占32.0%(68/150)；部分心电图检查异常率占28.21%(22/78)异常,主要表现在心率改变。89例检查胸片23例诊断为急性支气管炎或者支气管肺炎。治疗后除9例好转出院外,其余均治愈出院。转手足口病区的29例患儿经治疗均痊愈出院。结论：疱疹性咽峡炎部分可发展至手足口病,需临床多观察以便早期隔离治疗,以疱疹性咽峡炎为早期表现的手足口病预后良好。疱疹性咽峡炎易引起多脏器功能损伤,尤以心肌损害为多发,及早发现及早治疗能取得满意疗效,应引起高度重视。
목적：총결분석150례포진성인협염적림상증상체정이급치료전귀。방법：수집2011년3월지2013년4월아원주원포진성인협염환인150례적림상자료병진행회고성총결급분석。결과：150례중유29례발전성위수족구병,1례위중증；전부진행심기매검사,이상솔85.33%(128/150),기중CKMB이상자67례점52.34%；경과영양심기치료7~10천후복사심기매24례정상,계속치료지20천후재차복사32례회복정상。전부진행간공능검측,26례전안매승고,점32.0%(68/150)；부분심전도검사이상솔점28.21%(22/78)이상,주요표현재심솔개변。89례검사흉편23례진단위급성지기관염혹자지기관폐염。치료후제9례호전출원외,기여균치유출원。전수족구병구적29례환인경치료균전유출원。결론：포진성인협염부분가발전지수족구병,수림상다관찰이편조기격리치료,이포진성인협염위조기표현적수족구병예후량호。포진성인협염역인기다장기공능손상,우이심기손해위다발,급조발현급조치료능취득만의료효,응인기고도중시。
Objective:To summarize and analyze the clinical features and prognosis of 150 cases of herpangina. Methods:The clinical data of 150 cases of herpangina children who were admitted into our hospital from Mar. 2011 to Mar. 2013 were retrospectively summarized and analyzed. Results:Among 150 children, 29 cases were hand-foot-mouth disease(HFM)(1 severe case). The abnormal rate of myocardial enzyme (ME) inspection was 85.33%(128/150), among them, CKMB was 52.34%(67 cases). The ME of 24 cases recovered after 7-10 days of myocardial nutrient, 32 cases recovered after continued treatment to 20 days. For the aspect of liver function inspection, the transaminase of 26 cases were increased (32%, 68/150). Part of ECG abnormal rate was 28.21%(22/78), mainly displays in heart rate change. 23 cases were diagnosed with acute bronchitis or bronchitis or bronchitis. All children were discharged from hospital (9 cases were improved and the rest was recovered), meanwhile, the children who were transferred to HFM ward were also discharged. Conclusion:Herpangina can develop into HFM, so more observation and early isolation should be applied. The herpangina early manifested HFM has a good prognosis, while herpangina can induce multiple organ function injury, especially with myocardial damage, early detection and treatment could improve the clinical effect and high attention should be paid.
