目的:对儿童双侧扁桃体分别采用单极电刀凝切与传统剥离切除两种术式,利用Image-proplus(IPP)图像分析软件观察术后扁桃体深面血管管径的变化,为选择扁桃体术中及术后止血方法提供理论依据。方法对80例需行双侧扁桃体切除术的患儿,以随机方法对一侧扁桃体采用单极电刀凝切术切除,另一侧扁桃体采用传统剥离术切除。按照手术方式将其分为电刀凝切组与传统剥离组,取两组扁桃体标本的深面将其包埋、切片,HE染色后,在光学显微镜下用图像分析软件IPP测量扁桃体动脉,静脉血管管径,并对两组数据进行统计分析。结果电刀凝切组的扁桃体深面静脉小于传统剥离组的深面静脉,差异有统计学意义(P<0.05);电刀凝切组与剥离组的扁桃体深面动脉管径相比,差异无统计学意义(P>0.05)。结论电刀凝切组的扁桃体深面静脉血管管径小于传统剥离组,电刀凝切组的扁桃体深面动脉管径与传统剥离组的血管管径相比,差别不大。

목적:대인동쌍측편도체분별채용단겁전도응절여전통박리절제량충술식,이용Image-proplus(IPP)도상분석연건관찰술후편도체심면혈관관경적변화,위선택편도체술중급술후지혈방법제공이론의거。방법대80례수행쌍측편도체절제술적환인,이수궤방법대일측편도체채용단겁전도응절술절제,령일측편도체채용전통박리술절제。안조수술방식장기분위전도응절조여전통박리조,취량조편도체표본적심면장기포매、절편,HE염색후,재광학현미경하용도상분석연건IPP측량편도체동맥,정맥혈관관경,병대량조수거진행통계분석。결과전도응절조적편도체심면정맥소우전통박리조적심면정맥,차이유통계학의의(P<0.05);전도응절조여박리조적편도체심면동맥관경상비,차이무통계학의의(P>0.05)。결론전도응절조적편도체심면정맥혈관관경소우전통박리조,전도응절조적편도체심면동맥관경여전통박리조적혈관관경상비,차별불대。
Objective To provide theoretical evidence for selecting hemostasis method during and after surgery via two surgical methods of monopole electrocoagulation and conventional tonsillectomy which were applied respectively to remove bilateral tonsils for children and via image analysis software of Image-proplus (IPP) which was used to observe the postoperative changes of deep surface vascular diameters of tonsils. Methods 80 children patients who needed to be received tonsillectomy of bilateral tonsils were randomly received monopole electrocoagulation for one side of tonsils and received conventional dissection for the other side. The patients were assigned to the group of electrocoagulation and the group of conventional dissection.Deep surfaces of the two groups'tonsil samples were selected,embedded,sectioned,and dyed by HE.The arterial and venous diameters of tonsils were measured via IPP under optical microscope.The statistics obtained from the two groups were then analyzed. Results The diameter of deep surface veins in the group of electrocoagulation was shorter than that in the group of conventional dissection,and the difference was statistically significant (P<0.05).The difference of the diameters of deep surface arteries between the two groups was not statistically significant (P>0.05). Conclusion The diameter of deep surface veins of tonsils in the group of electrocoagulation was shorter than that in the group of conventional dissection, and the difference of the diameters of deep surface arteries between the two groups was not significant.
