目的:探讨凋亡调控因子Fas、FasL及caspase-3在肝细胞癌(HCC)组织中的表达及其临床意义。方法应用免疫组织化学SP法检测80例HCC患者肝癌组织及其相应癌旁正常肝组织中Fas、FasL及caspase-3的表达,并分析其表达与 HCC临床病理因素的相关性。结果 Fas、FasL及caspase-3在 HCC、癌旁组织中的表达比较,均差异有统计学意义(P<0.05)。在TNM 分期(Ⅲ~Ⅳ期)、有门静脉癌栓、有淋巴结转移的肝癌组织中Fas 的阳性表达率明显降低(P<0.05);在病理分级(Ⅲ~Ⅳ级)、TNM分期(Ⅲ~Ⅳ期)、有门静脉癌栓、有淋巴结转移的肝癌组织中FasL的阳性表达率明显增高(P<0.05);在病理分级(Ⅲ~Ⅳ级)、TNM分期(Ⅲ~Ⅳ期)、有淋巴结转移的肝癌组织中 caspase-3的阳性表达率明显降低(P<0.05)。80例 HCC患者肝癌组织中Fas、FasL与caspase-3的表达,两两之间并无明显关联性(狉=0.057,P>0.05)。结论 Fas/FasL系统的失衡及caspase-3低表达导致肝癌细胞凋亡障碍,在 HCC发生、发展及转移中起着重要作用。

목적:탐토조망조공인자Fas、FasL급caspase-3재간세포암(HCC)조직중적표체급기림상의의。방법응용면역조직화학SP법검측80례HCC환자간암조직급기상응암방정상간조직중Fas、FasL급caspase-3적표체,병분석기표체여 HCC림상병리인소적상관성。결과 Fas、FasL급caspase-3재 HCC、암방조직중적표체비교,균차이유통계학의의(P<0.05)。재TNM 분기(Ⅲ~Ⅳ기)、유문정맥암전、유림파결전이적간암조직중Fas 적양성표체솔명현강저(P<0.05);재병리분급(Ⅲ~Ⅳ급)、TNM분기(Ⅲ~Ⅳ기)、유문정맥암전、유림파결전이적간암조직중FasL적양성표체솔명현증고(P<0.05);재병리분급(Ⅲ~Ⅳ급)、TNM분기(Ⅲ~Ⅳ기)、유림파결전이적간암조직중 caspase-3적양성표체솔명현강저(P<0.05)。80례 HCC환자간암조직중Fas、FasL여caspase-3적표체,량량지간병무명현관련성(비=0.057,P>0.05)。결론 Fas/FasL계통적실형급caspase-3저표체도치간암세포조망장애,재 HCC발생、발전급전이중기착중요작용。
Objective To investigate the expression and clinical significance of apoptosis regulatory factors Fas, FasL and caspase-3 in hepatocellular carcinoma (HCC)tissues. Methods SP immunohistochemistry was used to detect the expression of Fas,FasL and caspase-3 in HCC tissues and the corresponding para-tumor normal liver tissues of 80 cases. The correlation between the expression and clinical pathological factors of HCC was analyzed.Results Compared to para-tumor tissues,the expression of Fas,FasL and caspase-3 in HCC was statistically significant difference (t=2.34,3.53,6.28,respectively,P<0.05). The positive expression rate of Fas was significantly reduced in HCC tissues of TNM staging Ⅲ~Ⅳ with portal vein tumor thrombus and lymphatic metastasis (t= 6.65,P<0.05). There was an obvious increase of positive expression rate of FasL in HCC tissues of pathological grading Ⅲ~Ⅳand TNM staging Ⅲ~Ⅳwith portal vein tumor thrombus and lymphatic metastasis (t= 7.13,P<0.05). Besides,a significant decrease of the positive expression rate of caspase-3 was observed in HCC tissues of pathological grading Ⅲ~Ⅳ and TNM staging Ⅲ~Ⅳwith lymphatic metastasis (t= 3.96,P<0.05). No obvious correlation existed between any two of the three factors (r= 0.057,P>0.05).Conclusions he imbalance of Fas/FasL systems and lower expression of caspase-3 may contribute to the apoptosis disorder of HCC cells,which plays an important role in the occurrence,development and metastasis of HCC.
