目的：探讨标准大骨瓣开颅术治疗重型颅脑损伤的方法及临床效果。方法：分析我院2006年~2012年实施美国标准外伤大骨瓣开颅术治疗的79例重型颅脑损伤患者的临床资料。结果：标准外伤大骨瓣开颅术治疗重型颅脑损伤患者79例，其中额颞顶部广泛脑挫裂伤56例，合并硬膜下血肿49例，合并有硬膜外血肿17例，合并脑内血肿23例，术后按 GOS评定恢复良好36例，中残15例，重残6例，植物生存2例，死亡20例。结论：标准外伤大骨瓣开颅术能清除幕上颅内血肿和坏死脑组织，显露彻底，减压充分，提高抢救成活率，降低死亡率和致残率，疗效确切，是治疗重型颅脑损伤的有效方法，值得在临床工作中加以推广。

목적：탐토표준대골판개로술치료중형로뇌손상적방법급림상효과。방법：분석아원2006년~2012년실시미국표준외상대골판개로술치료적79례중형로뇌손상환자적림상자료。결과：표준외상대골판개로술치료중형로뇌손상환자79례，기중액섭정부엄범뇌좌렬상56례，합병경막하혈종49례，합병유경막외혈종17례，합병뇌내혈종23례，술후안 GOS평정회복량호36례，중잔15례，중잔6례，식물생존2례，사망20례。결론：표준외상대골판개로술능청제막상로내혈종화배사뇌조직，현로철저，감압충분，제고창구성활솔，강저사망솔화치잔솔，료효학절，시치료중형로뇌손상적유효방법，치득재림상공작중가이추엄。
Objective:To explore the ef ect of standard large trauma craniotomy in severe craniocerebral injury.Methods:79 severe craniocerebral injury patients were adopt standard large trauma craniotoG my during 2006 ~2012 year.Result:79 cases with severe craniocerebral injury performed standard large trauma craniotomy,including 56 cases with severe frontal-parietal-temporal brain trauma,49 comG bined with subdural hematoma,17 combined with epidural hematoma,23 combined with introcerebral hematoma.Al cases were evaluated by GOS after operation,36 cases cured,15 moderate disability,6 severe disability,2 vegetative state and 20 cases died.Conclusion:Standard large trauma craniotomy reveals and de-compresses thoroughly during operation,so make it easy to handle for supratentorial heG matoma and necrosis tissue.It can be ameliorate prognosis,abate the sequel and improve the outcome of severe traumatic brain injured patients significantly.
