目的了解东莞市麻涌镇农村65岁以上老年人群高血压患病现状,探讨体质指数与高血压的关系,为农村老年人群高血压的预防与控制提供流行病学依据。方法于2010年11月~2011年1月,以在东莞市麻涌镇各村社区卫生服务站就诊的65岁以上老年人群为研究对象,并进行血压、身高和体质量的测量。采用描述性分析、t检验、卡方检验和多因素 Logistic 回归等方法对资料进行分析。结果共有2004名65岁以上老年人接受检查,其中男性827人(41.26%),女性1177人(58.74%),男女性别比1暶1.42；平均年龄72.44暲6.06岁；高血压粗患病率为44.36%,标化患病率为44.62%,男性粗患病率为44.26%,女性为44.44%,不同性别高血压患病率差异无统计学意义(P =0.94)；超重率为32.09%,肥胖率为11.73%,女性超重率和肥胖率(33.56%,13.26%)均高于男性(29.99%,9.55%),差异有统计学意义(P <0.01)；超重组高血压患病率为52.10%,肥胖组高血压患病率为56.60%,均显著高于 BMI 正常组(37.61%)；多因素结果显示,超重组和肥胖组患高血压的风险分别是 BMI 正常组的1.83和2.16倍。结论东莞市麻涌镇农村老年人群高血压患病率较高,超重和肥胖是高血压患病的重要危险因素。

Objectives To investigate the prevalence of hypertension among rural elderly population aged over 65 years old in Machong town,Dongguan City,to analyze the relationship between body mass index (BMI)and hypertension and to provide the epidemiological evidence for prevention and control of rural elderly population.Methods The rural elderly pop-ulation aged over 65 that visited the village community health service stations in Machong Town,Dongguan City were ran-domly selected during October 2010 and February 2011 and accepted physical examination to get the data of blood pres-sure,height and weight information.Descriptive statistical analysis,chi-square test and multiple logistic regression analysis were used in the analysis.Results A total of 2004 participants were included,among which 827(41.26%) were men and 1 1 77(58.74%)were women,the average age was 72.44±6.06.The prevalence of hypertension was 44.36%,the standardized prevalence was 44.62%,and 44.26% for male and 44.44% for female.There was no significant statistic difference between gender (P =0.94).The rates of overweight and obesity were 32.09% and 11.73%. The rate of overweight and obesity for the female (33.56%,13.26%)was higher than that of the male (29.99%, 9.55%),and the difference was statistically significant (P <0.01).The incidence rate of hypertension in overweight (52.10%)and obesity (56.60%)was significantly larger than that in normal weight population (37.61%).Multi-logistic analysis indicated that the risk of hypertension in overweight and obesity group was 1.83 and 2.1 6 times higher than that of the normal BMI group.Conclusions The prevalence rate of hypertension in elderly of Machong Town was high.Overweight and obesity are important risk factors of hypertension.
